Lake Park is a more than 250-bed long-term skilled nursing care facility located in Sylvania, Ohio. It is a member of ProMedica Health System and is certified by the Ohio Department of Health to meet Medicare, skilled Medicaid and participation requirements. Its rehabilitation services include physical, speech, occupational, recreational and respiratory therapies. The organization s staff includes social workers experienced in geriatric care and a chaplain who provides guidance and counseling services.
