Ovarian Cancer
Mutations of the BRCA1 and BRCA2 genes are the most common. Women with a BRCA1 mutation have up to a 55-65% risk of developing breast cancer in their lifetimes and BRCA2 carriers have up to a 45% lifetime risk. An increased ovarian cancer risk is also associated with these genetic mutations.
Cancer Care

Ultrasound
After completing my Breast cancer treatment, in 2013 it was time to retire. I then began to ask about breast surgeons of neighbors and Dr Gardner was highly recommended. When I arrived in the office I felt like I was at home. The ultrasound tech was king and caring and did her best to relax me. Dr Gardner started our conversation with “you may be in Florida, but you have a New York surgeon. They were both thorough, compassionate and caring. MRI
Magnetic resonance imaging (MRI) is an imaging tool that creates detailed, cross-sectional pictures of the inside of the body. Using radiofrequency waves, powerful magnets and a computer, MRI systems are able to distinguish between normal and diseased tissue.
Radiology
A yearly mammogram, beginning at age 40 (except in unusual circumstances) is your best assurance against advanced stages of breast cancer. The baseline for evaluation should include two views of each breast. In the United States, mammograms should be performed at facilities certified by the American College of Radiology. A mammogram can detect an abnormality in the breast three to four years before physical symptoms are even present. This earlier detection has been proven to reduce the death rate from breast cancer by approximately 30%. Fully 70% of all women diagnosed with breast cancer had no known risk before the time of diagnosis. Mammography is the current “gold standard” for the early detection of breast cancer. Still, newer techniques and emerging technologies are in development and limited use that will produce superior imaging at an even earlier stage than the standard mammogram.
X-Rays
Mammography
A mammogram is an X-ray of the breast. It is usually 2 views that are taken of each breast from different angles. Mammography is obtained by a special machine that uses a lower dose of radiation than the usual x-ray.
Computed Tomography
Breast MRI plays an important role in cancer diagnosis, staging and treatment planning. With MRI, we can distinguish between normal and diseased tissue to precisely pinpoint cancerous cells within the breast. It is also useful for revealing metastases. MRI provides greater contrast within the soft tissues of the body than a CT scan.
Chemotherapy
When we get the final pathology report back, we can discuss if she’ll benefit from hormonal therapy, and in certain very select cases, chemotherapy, which is required less and less as we get into very early detection and diagnosis.
Radiation Therapy
Some patients will be candidates for what is called a boost in the operating room. This is a dose of 10 Gray (e-IORT) to the area surrounding where the cancer used to be for the purpose of reducing recurrences. This has been shown to be extremely effective in reducing local recurrence. In such cases, a short course of abbreviated whole breast radiation therapy is typically offered as well and can be done near where the patient lives.

Wound Care
A persistent irritation or sore of the nipple may be a benign skin condition that will clear up with typical dermatological treatment. The most common mistake is to ignore a sore on the nipple that is accompanied by a bloody or non-bloody discharge for several months. If the sore has not healed in three to four weeks after commencing the use of topical wound care including antibiotics, then you should consult with a qualified breast clinician to rule out the beginnings of Paget’s disease. Paget’s disease is a particular nipple disorder associated with cancer in the breast tissue immediately underneath the nipple. The breast clinician will require a biopsy to determine whether the condition is cancerous.
Vascular Surgery
In 1973, Dr. Gardner relocated to Palm Beach County to join Palm Beach Medical Group (joining the medical society in 1974) where he practiced general and vascular surgery until 1991. In 1991, he “came home to my one true love, which is breast issues” and founded his private practice, Palm Beach Breast Institute in West Palm Beach.

Plastic Surgeons

Plastic Surgery
As of July 2014, Dr. Gardner is now offering electron intraoperative radiation therapy (abbreviated e-IORT) for breast cancer treatment. This is a major breakthrough in breast cancer treatment because in patient’s who qualify for the treatment by virtue of tumor size, age and so on, treatment by surgery and radiation therapy may be completed in a day. With today’s new treatment, while a patient is already in the hospital to have a cancer removed the patient can also receive about a two minute dose of high-energy electron intraoperative radiation therapy to the breast tissue near where the breast cancer used to exist. The breast is reformed with onco plastic surgery and then the non-irradiated skin is closed and the patient can go home. In many breast cancer cases, surgery and radiation therapy are done in a day under anesthesia. Please note that we use stainless steel protectors behind the breast as we radiate through the surrounding breast tissue to protect the heart, lung and ribs, so these do not get any radiation. This stainless steel disc stops the radiation completely from penetrating the other organs. Typically, a sentinel lymph node is sampled at the same time under the same anesthesia. The total operation takes about two hours to do and that includes the set up and delivery of the radiation therapy at surgery. The dose given is 21 Gray of radiation.
