Arthritis
), also known as a knee arthroplasty, is the most common joint replacement surgery performed in the United States. Since it’s advent in the late 1960's, millions of patients with osteoarthritis, rheumatoid arthritis, and post traumatic arthritis have benefited from knee replacements. Historically, knee replacements were only indicated in patients older than 65 years with end stage osteoarthritis. Technological advances and procedural improvements have expanded these indications to include younger patients with a variety of diseases involving the knee. Many patients with night pain, pain at rest, difficulty performing ADL’s or the inability to enjoy their customary hobbies and sporting activities are excellent candidates for a total knee replacement.
Primary Care
You will need a pre-surgical evaluation before surgery. While we will perform our own pre-surgical evaluation (see PAT below), you may need to schedule an appointment with your primary care physician for pre surgical evaluation and clearance particularly if you have any medical problems.

It is done with the patient lying on the side. The patients are monitored with EKG, blood pressure cuff and blood oxygen-monitoring device. The skin is cleaned with antiseptic solution and then the procedure is carried out. X-ray (fluoroscopy) is used to guide the needle for inserting the tubing.

Once you and your physician have decided that you will need eye surgery, our surgical coordinator will be notified. For elective surgery, we will be calling you within 1-2 weeks to schedule a convenient date for surgery. If you do not hear from us within 2 weeks please call 415-642-0707. The discs are soft, cushion-like pads which separate the hard vertebral bones of your spine. A disc may be painful when it bulges, herniates, tears, or degenerates and may cause pain given the corresponding level. Other structures in your spine may also cause similar pain such as the muscles, joints and nerves. Usually, we have first determined that these other structures are not your sole pain source through history and physical examination, review of x-rays, CT/MRI, and/or other diagnostic injection procedures such as facet and sacroiliac joint injections and nerve root blocks before performing discography.

Be taken for a CT scan. You should not drive for eight hours following this procedure. You will wait 30-45 minutes after your CT scan to go home. You will be given, if desired, a prescription for pain medication over the next 2-3 days when your muscles may be sore. You can also use your existing pain medication.

Dr. Atkin, an avid medical volunteer, is the current President of Operation Rainbow, an organization providing free medical and surgical care to indigent children in developing countries. He has led volunteer teams from St. Luke's to the Philippines, Nicaragua, El Salvador, and the Caribbean for over a decade.

Orthopedic Surgery
The recent mission to Manta, Ecuador in June 2008 was an overwhelming success. We received great cooperation from our hosts including the use of 3 operating rooms. We evaluated over 200 children and chose representative teaching cases. Our three surgeons performed more than 60 surgeries with the local surgeons, to teach them modern techniques in orthopedic surgery. Our physical therapists, from Potrero PT, Mission District PT and CPMC were very active in teaching the local therapists.

Physical Therapy
Most rotator cuff injuries improve with prolonged physical therapy. Physicians also may advise non-steroidal anti-inflammatory drugs (NSAIDs) or steroid injections to decrease inflammation and pain. Any of the above rotator cuff problems may potentially benefit from surgery, however, and surgery is often required to repair a rotator cuff tear.
