        On May 12, 2017, Patient was released by OHSU to go to a rehab facility. He was notified of this by the attending physician before lunch time. Transport company "Community Ambulance" was notified by OHSU at about 1:00PM that the patient needed transport from OHSU to
        Rehab


        They were also informed at that time that the patient needed a heavy duty gurney. Community Ambulance gave OHSU a time that they would be able to pick the patient, 4:00 PM the same day. They did not meet their obligation. They finally arrived at 5:40 PM leaving the patient to wait for Community Ambulance convenience. This is not a rare occasion with this company and this company seems to not have any care for customer service or care.
