Hashimoto’s thyroiditis is the most common cause of an underactive thyroid gland (hypothyroidism). It is an autoimmune problem in which the body’s immune system attacks the thyroid gland. In rheumatoid arthritis the body’s immune system attacks the joints; in Hashimoto’s thyroiditis it pounces upon the thyroid gland. The inflammation causes the gland to extrude stored thyroid hormone. This results in symptoms of an overactive thyroid. Over time, the inflammation restricts the gland’s ability to make thyroid hormone, the person now has hypothyroidism.

Diabetic eye disease includes cataract, diabetic retinopathy and glaucoma. Cataract and glaucoma also affect people who do not have diabetes. A common type of diabetic retinopathy is non-proliferative diabetic retinopathy. With this problem, small blood vessels leak causing the retina to swell. This is macular edema. The result is blurred vision. It may occur in only a part of the vision as noted in the image.
Eye Exam
The good news: early detection by a dilated eye exam with timely treatment and follow-up can prevent vision loss and blindness. A mere vision test will not cut the mustard, you need to have your eyes dilated and carefully examined by a professional.

Gabapentin is often the first line of treatment for diabetic neuropathy pain. It has been around since 1993, originally approved for the treatment of epilepsy. The FDA warns that serious life-threatening and fatal respiratory problems may occur from the drug, especially if used at the same time as opioids or antidepressant/anxiety medications. Gabapentin is not an opioid and is not a controlled substance but requires a prescription. The nonopioid feature makes it a good starting point for diabetic polyneuropathy and other nerve related pain. The main treatment for diabetic neuropathy, however is control of the diabetes. Gabapentin and Lyrica work by reducing the excitement of nerves, one of the mechanisms of pain. The problem is that it works anywhere there are nerves including the brain. The most common reported side effects are dizziness, somnolence and swelling of the legs and feet. Only 20% of patients reported improvement in a study of patients taking gabapentin for diabetic neuropathy.

In Hashimoto’s thyroiditis, your immune system mistakenly identifies the thyroid gland as a threat and starts attacking it. This attack leads to inflammation and damage to the thyroid tissue. Over time, this can interfere with the thyroid’s ability to produce enough hormones, leading to an underactive thyroid or hypothyroidism.

Diabetic Retinopathy
Rybelsus must be taken once daily in the morning, at least 30 minutes before food or other medications and followed by a full glass of water. It has the same warnings as the injectable GLP1 products including the potential increased risk of thyroid c-cell tumors. People who have had medullary thyroid carcinoma (MTC) or who have a family member who has had MTC are advised not to use Rybelsus or any GLP1 product. There are also has warnings about pancreatitis (inflammation of the pancreas), diabetic retinopathy (damage to the eye’s retina), hypoglycemia (low blood sugar), acute kidney injury and hypersensitivity reactions.
Thyroid Cancer
Ne absorbed into the human body. If that iodine is radioactive, there is increased risk of thyroid cancer, especially in children. Radioiodine is formed close to us at the Savannah River site and the Alvin W. Vogtle Electric Generating Plant (about 10 miles away in GA) as a byproduct of nuclear reactive operations. Radioisotopes of iodine have been released to the atmosphere from the Savannah River site since 1955 according to a 1990 report from the US Department of Energy.

Ultrasound
If a nodule is discovered, the first tests are generally thyroid blood tests for inflammation, over or under activity of the gland. These are often normal. A thyroid ultrasound is likely the next test. The ultrasound can detect if there is a suspicious nature of the nodule needing to additional tests. This might include a scan of the thyroid after a small dose of radioiodine to determine if the nodule is functioning. If there is a suspicion of possible malignancy in the gland, a biopsy may be requested. This is called an FNA (fine-needle aspiration). In this test, a small amount of tissue is drawn from the nodule with a tiny needle and submitted for pathologic evaluation. Most often, there is no cancer. If no evidence of cancer, the recommendation is for periodic ultrasound follow-up to ensure the nodule has not changed or become more suspicious.
MRI
Thyroid nodules are often discovered during a routine physical examination or on tests as ultrasound, CAT scan or MRI done for non-thyroid reasons. Occasionally the patient will notice a lump in the neck while looking the mirror. There was a case in which a viewer noticed a lump on the neck of a TV personality, notified the person and it turned out thyroid cancer which was treated promptly.

*Recently the FDA issued a warning about necrotizing fasciitis of the perineum occurring in patients taking SGLT2 inhibitors. Necrotizing fasciitis, also called Fournier’s gangrene is an extremely rare but life-threatening bacterial infection of the tissues around the genitals. It generally occurs in males between ages 50 and 79 and is estimated to occur in a little over 3 patients per 100,000 taking this class of medication.

“Patients seeking treatment of peripheral neuropathy need to be aware of a recent controversial method that involves collaboration between chiropractors, medical doctors and/or podiatrists.... In most of the schemes identified, the patient will briefly see a medical doctor on the first visit. The medical doctor will order all the tests, braces and injections. He may recommend the patient undergo physical therapy and/or chiropractic care in the clinic as well. Typically, all of the localized anesthetic injections are given by a nurse. A chiropractor and/or physical therapist may render various treatments over the course of care, further increasing the cost. The patient rarely sees the medical doctor again....A typical PN scheme protocol includes expensive and sometimes painful, nerve conduction testing. These tests may be done at the onset of treatment and intermittently throughout the care program. These tests may have no basis other than to generate income for the clinic. The clinician may X-Ray the patient’s neck, back, arms, legs and feet. These x-rays have often been found to have no value in helping the doctor reach a diagnosis. The clinic also orders a variety of elastic and hard, plastic braces for the patient’s neck, back, wrists and/or ankles. In the majority of instances, these braces were found to offer little assistance to the patient’s painful condition. These supports are expensive with bills totaling over thousands of dollars.
