Life Care Centers of America

My mom was in several rehab/nursing facilities in & around the Jacksonville area b4 she passed away. Currently, my dad is at Life Care Center to build himself back up b4 he comes home. I wish I would have known about Life Care when my mom was still alive. This place is the best!! The staff r wonderful, the facility immaculate, the food is great, & they do so much 4 the residents!! they even have an ice cream parlor!! No kidding!! If u or a relative must stay at a rehab/ nursing facility 4 any amount of time this is THE place!!

The entire staff at Life Care is wonderful. They are caring and helpful. I've never been to this facility without being greeted cheerfully. They take excellent care of all their patients. I would highly recommend Life Care at Wells Crossing if you need rehab or plan on staying long-term.

If you value your loved one, skip this place. They have nurses like Danita that cares more about filling out her paperwork than attending to the needs of a patient even during a crisis. A CNA like Phyliss that likes to taunt and tease a resident that cannot defend herself. It's a real shame, that because of present legislation that they get away with this. I cannot phathom what i witnessed in this place.
