Plantar Fasciitis
Plantar fasciitis is more of a misnomer, since “itis” means inflammation, and most patients who have been diagnosed with plantar fasciitis actually have a weakened, degenerated plantar fascia.
Rheumatology
A 2019 study in the Journal of clinical rheumatology ( 10 ) compared compared periarticular (neurofascial) dextrose prolotherapy versus physiotherapy for treatment of chronic rotator cuff tendinopathy in the short term. The results: Neurofascial dextrose was more effective than physiotherapy for alleviating pain in 2 weeks and they were similar 3 months after the interventions. For disability, dextrose was more effective than physiotherapy 2 weeks and 3 months after the interventions. However, the changes in the physiotherapy group seemed to be more sustained.

My parents instilled in me a love for learning and a desire to serve others. Thus, I chose nursing as my career. I received my nursing degree at Hutchinson Community College where I learned how to put my passion for people into practice. My nursing career began in a local hospital where I worked in a Medical/Surgical unit. Here I was able to gain a great overview of general medicine and surgery and learn more about how to care for a wide variety of patient issues. I then moved to working in Pediatrics which was a fun experience, but also heart-wrenching at times, because it is not easy to watch children suffer.
Primary Care
Philip had been treated successfully by the providers of Caring Medical for multiple injuries over the course of 8 years. So, when pain began after hyperextending his right thumb, he knew where to turn after his primary care physician told him that nothing could be done due to the degeneration of the joint. Philip experienced moderate thumb pain while gripping and lifting objects which had begun limiting his exercise routine. Upon physical exam in our office, the provider confirmed instability of the CMC and MCP joints and recommended a series of H3 Prolotherapy treatments using a traditional dextrose solution with added proliferants. Philip was advised to continue with normal activity, except for pushups. A brace was recommended during periods of activity for added stability. After his first treatment, Philip reported 50% pain relief in CMC joint and 25% pain relief in MCP joint. When he returned for his third treatment, the MCP joint pain was completely resolved and he was able to reduce time wearing the brace without any increased pain. During his fourth and final visit, Philip reported 99% pain relief in his thumb and was able to return to full activity.

Heart rate variability (HRV) refers to the variability between successive heart beats, specifically the R-R intervals on an EKG recording. If a person has a heart rate of 60 beats per minute, the average R-R interval would be 1 second but some R-R intervals may be 0.8 seconds and others 0.12 seconds.

Labor and Delivery
BP came to Caring Medical after suffering from pelvic floor pain for 14 months in her late twenties. Her pain had started immediately following childbirth and became progressively worse despite other treatments. Brittney had tried massage, acupuncture, physical therapy, and dry needling without relief. She experienced constant pain and muscle spasms that would worsen to the point of being unable to carry her child. Among other symptoms, Brittney experienced constipation, pain with urination, and rectal prolapse. Gastroenterology
Clinical and Experimental Gastroenterology built on this research by suggesting that managing side effects, in this case, constipation would help the patient with their failed back problems. Here is what the researchers wrote: “Opioids are an effective treatment for moderate-to-severe pain. However, they are associated with a number of gastrointestinal side effects, most commonly constipation. Laxatives do not target the underlying mechanism of opioid-induced constipation, so many patients do not have their symptoms resolved. Fixed-dose prolonged-release oxycodone/naloxone tablets contain the opioid agonist oxycodone and the opioid antagonist naloxone. Naloxone blocks the action of oxycodone in the gut without compromising its analgesic effects.

EK first began experiencing the symptoms of Ehlers-Danlos Syndrome, Hypermobility type in the fifth grade, when one of her knees subluxed. Over the next 12 years, the pain and joint subluxations spread to other joints including the other knee, elbows, shoulders, and spine. EK tried many different forms of therapy including physical therapy, massage, ultrasound, taping, and compression braces which managed her pain well enough to perform daily activities as well as gymnastics, track, and cross country. At the age of 19, she tore the meniscus in her right knee and underwent surgical meniscus repair. Following the operation, she experienced intense pain, and subsequently underwent a second operation. While the symptoms in her knee appeared to be resolved, pain in her other joints persisted. During this time, EK also began experiencing other health issues including hypothyroidism, eczema, chest pains, food allergies, irregular menstrual periods, and degenerative disc pain in her neck and back.

Ultrasound
Using high-resolution ultrasound, the diameters of the right and left vagus nerves in the neck are obtained. When the vagus nerves are degenerating, meaning neurons within the vagus nerves are dying, the diameter and the cross-sectional area diminish (much like a muscle that is atrophying). With proper treatment, the size of the vagus nerves improves.

Test Results: If you have MRI, X-ray, CT, EMG, DMX, or other pertinent lab reports, please bring them with you to the appointment. If you have sent us your reports ahead of time, our team will ensure those reports are put into your chart.
Radiology
Once in position, the person puts his or her neck, spine or peripheral joint through a series of motions while the DMX is videotaping the bone movement. Digital Motion X-ray is similar to a movie camera. The camera takes 30 individual x-ray frames per second to create the motion x-ray. The successive x-rays are digitized and sequenced to create a video representation of the movement of the bones. Sometimes, additional views are done where the joint is put under stress: by the person holding a weight, by the person doing a specific exercise, or by the radiology technician or physician putting passive pressure in a certain direction to help document the instability.

Additionally, stem cell treatments are touted as a one-shot wonder. Please understand that there are no shortcuts to treatment, especially something as serious as osteoarthritis. If someone tries to give you one, they are just plain ol’ wrong. Your joint is literally dying (think “necrosis” or “gangrene”). Frozen Shoulder
The shoulder is one of the most mobile joints in the body and also one of the most unstable. Shoulder instability due to ligament laxity is the primary cause of the majority of shoulder pain conditions, yet goes undiagnosed in many cases. Eighty percent of chronic elbow pain is due to a sprain of the annular ligament. This ligament is rarely examined by a family physician or an orthopedic surgeon. Nearly all of our patients with chronic elbow pain tell us their doctors told them they have tennis elbow. Tennis elbow is also known as lateral epicondylitis. The latest treatment for this condition is the dreaded cortisone shot! Cortisone weakens tissue, whereas Prolotherapy strengthens tissue. Cortisone has temporary effects in regard to pain control whereas Prolotherapy has permanent effects. However, cortisone does have one permanent effect: continual use will permanently weaken tissue. Anyone receiving long-term prednisone or cortisone shots will confirm this fact.
Slipped Disc
What your MRI shows is the results of joint instability. That slipped disc or herniated disc is caused by instability! If your ligaments were tight, the disc would not be able to come out of place. Once your ligaments become stretched out, the disc is able to slip out of position. MRI’s are a good tool for certain things, but since you are not moving during an MRI, it does not show the instability in a joint. Digital Motion X-ray and ultrasound are good tools for showing instability because they show the joints while they are moving and you can see the instability.

MD, a 59 year old school administrator from Alaska, had chronic right lateral ankle pain for three years after a severe ankle sprain. Initial x-rays were unremarkable. Her ankle inverted while dancing, significantly increasing her pain. She was treated with an extensive period of guarded weight bearing, crutches, and non-weight bearing for several months but noted no improvement. A corticosteroid shot produced no results. An MRI on 7/7/2010 then showed prominent AVN of the talar dome involving the central to posterior aspect. She was offered various surgical options including arthroscopy with debridement, allograft osteochondral transfer, core decompression and ankle fusion through external fixation. The prognosis for complete pain relief with these options was guarded so MD decided to seek more conservative treatment.

A March 2021 review study ( 38 ) looked at research outcomes for the treatment of Adductor-related groin pain. They found that the intervention(s) showing the greater level of strength of evidence, and the greater grade of recommendation are compression clothing therapy, manual therapy and strengthening exercise, and prolotherapy.”
TMJ Dysfunction
Temporomandibular Joint Dysfunction can cause a tremendous amount of pain in the jaw, face, head, and neck, as well as compromise a person’s ability to talk, chew, and function normally. During this time, many people find they are merely chasing symptoms, trying everything from dental splints and pain pills to more drastic dental and surgical procedures. However, to stop this cycle, it is necessary to look further. Our providers work with patients to find underlying causes of the pain and often find that they were missing a critical TMJ instability diagnosis which is why responses to other treatments were short lived.
Orthotics and Prosthetic Therapy

Achilles Tendonitis
NA had been dealing with chronic Achilles tendonitis for more than three years prior to being seen at Caring Medical. At 40 years old, he was normally very active, but had to stop running and playing tennis due to pain. After the first two dextrose Prolotherapy treatments, he saw improvement in daily pain levels but he still had pain with exercise. NA was advised to wear a boot for two months while we continued to treat with the addition of Platelet Rich Plasma (PRP Prolotherapy) for his following four treatments. After the series of treatments, he reported significant improvement which was also visible when viewing the tendon under ultrasound!
Ankle Instability

Ankle Sprain
JC came to Caring Medical, approximately 1 month after spraining his ankle. Although he was not experiencing severe pain, the ankle became weak and greatly limited his ability to exercise. He had suffered chronic ankle injuries in his 20s and knew that the statistics for re-injury with ankle sprains are very high. As a runner, now in his early 50’s he did not want to chance further injury or eventual ankle surgery, and he really wanted to get back to running strong. After 5 treatments with Prolotherapy and Platelet Rich Plasma, he was able to return to trail running 16 miles without any pain or weakness. JC said he felt better than he did in high school!

Chronic foot pain is relatively common and almost expected, as the feet bear tons of force every day just from the process of walking. Often, chronic foot pain begins from a collapse of the medial arch. This occurs because the spring ligaments and plantar fascia can no longer support the arch. Treatments, such as arch supports, may provide temporary benefit but unless the ligament strength is restored, most patients find that they circle through various orthotics, cortisone injections, pain pills and dozens of new shoes trying to cushion the foot. However, Prolotherapy injections to strengthen the arch can provide more permanent results. Another common long-term issue that arises from ligament laxity is heel spurs. Remember, bone spurs are the long-term reaction to joint instability. Heel spurs are due to the weakened ligamentous support of the plantar fascia. Prolotherapy injections, which may include cellular solutions such as Platelet Rich Plasma, to strengthen the plantar fascia will eliminate chronic heel pain. There is generally not a need for heel spurs to be surgically removed after the supportive ligaments and plantar fascia have been repaired.

At her February 2010 visit, the pubic symphysis was treated again, as well as the right hip. At this visit, JD noted a new pain in her lower right back which was also treated with dextrose Prolotherapy with sodium morrhuate at that time. Over the course of the next year, JD was seen in the office three times (including seven months between two of the visits) necessitating treatment to her left knee, right hip, and new-onset metatarsalgia of her left foot. To date, her disabling groin pain is down to a manageable level, but feels that some of the pain is secondary to the two meshes she has in her. JD is back to work, but not full time like we had hoped.
