Unlike osteoarthritis which follows a predictable pattern in certain joints, rheumatoid arthritis is a system-wide disease. It is an inflammatory disease where the patient's own immune system attacks and destroys cartilage. The exact cause of rheumatoid arthritis is not known. Although it is not an inherited disease, researchers believe that some people have genes that make them more susceptible. There is usually a "trigger," such as an infection or environmental factor, which activates the genes. When the body is exposed to this trigger, the immune system begins to produce substances that attack the joint. This is what may lead to the development of rheumatoid arthritis.

Ultrasound
Often used to provide relief of the inflammation and from bunion pain. Ultrasound therapy is a popular technique for treating bunions and their associated soft tissue involvement.
MRI
A stress fracture of the metatarsals (the bones in the middle of the foot) can occur when a person walks or runs excessively. Putting full weight on the foot causes increased pain. The affected area on the metatarsal bone is tender to touch. Stress fractures may not be seen on x-rays if they are small or new (in an early stage). Sometimes CT, MRI, or bone scanning shows the fracture when x-rays do not. When a developing stress fracture is recognized early, stopping activities that aggravate the fracture may be all that is necessary. In more advanced and severe cases, crutches and a cast are necessary.

Fractures of the foot and ankle are caused by falls, twisting injuries, or direct impact of the foot against hard objects. Foot fractures are common and cause considerable pain, which is always made worse by attempting to walk or put weight on the foot. Diagnosis is usually made by x-ray. Sometimes, computed tomography (CT) or magnetic resonance imaging (MRI) is required. Treatment varies with the bone involved and the fracture type but usually involves placing the foot and ankle in a cast. Bunion Surgery
Bunion surgery is NOT more painful than other surgeries: Any surgery and the recovery period will have pain associated with it. However, bunion surgery is not more painful than other surgeries. Podiatrist will work closely with his patient to provide pain medication and rehabilitation that will minimize your pain.
Joint Replacement
In arthroplasty, the damaged ankle joint is replaced with an artificial implant (prosthesis). Although not as common as as total hip or knee joint replacement, advances in implant design have made ankle replacement a feasible option for many people. In addition to providing pain relief from arthritis, ankle replacements offer patients better mobility and movement compared to fusion. By allowing motion at the formerly arthritic joint, less stress is transferred to the adjacent joints. Less stress results in reduced occurrence of adjacent joint arthritis. Ankle replacement is most often recommended for patients with: advanced arthritis of the ankle, destroyed ankle joint surfaces and an ankle condition that interferes with daily activities.
Bunions
This surgery will remove the bony enlargement, restore the normal alignment of the toe joint, and relieve pain. A simple bunionectomy, in which only the bony prominence is removed, may be used for the less severe deformity. Severe bunions may require a more involved procedure, which includes cutting the bone and realigning the joint. Recuperation takes time, and swelling and some discomfort are common for several weeks following surgery. Pain, however, is easily managed with medications prescribed by your podiatric surgeon.
Gangrene
Damage to blood vessels and impairment of the immune system from diabetes make it difficult to heal these wounds. Bacterial infection of the skin, connective tissues, muscles, and bones can then occur. These infections can develop into gangrene. Because of the poor blood flow, antibiotics cannot get to the site of the infection easily. Often, the only treatment for this is amputation of the foot or leg. If the infection spreads to the bloodstream, this process can be life-threatening.
Ingrown Toenails
An ingrown toenail is when the edges of a toenail grow and pierce into the soft tissue of the toe. This condition is most common on the big toe (thumb) and causes quite a bit of pain, swelling, redness, and when left untreated, an infection. Patients with poor circulation, such as diabetics and peripheral vascular disease are more likely to have complications from ingrown toenails. See our Ingrown Toenail NYC Patient Handout.

The laser we utilize is FDA-cleared as a laser for use in podiatry, dermatology and plastic surgery. However, the treatment of nail fungus is an off-label use. This means that the laser has not been approved by the FDA specifically for the treatment of nail fungus. The company has submitted data to the FDA in the hope of getting approval, however there has been no approval for nail fungus treatment to date. Corns
Corns and calluses are hardened / thickened skin areas. Corns commonly develop between the toes or on top of the toes. Calluses usually develop on the bottoms (soles) of the feet, although they can form on the sides or on the toes, as well. The difference between corns and calluses is that corns have a nucleus (hard center) and callus is diffused. Corns and calluses are very common foot conditions, especially in older people. These conditions can start out being annoying at first, but can become very painful if left untreated. Corns & calluses can become inflamed and infected, especially in people with poor blood flow to the feet. Corns and calluses are often preventable with proper footwear, orthotics, etc. When they do form, however, these conditions respond well to treatment.

If a corn or callus becomes very painful or inflamed, see your foot doctor. If you have diabetes or poor circulation, call your podiatrist. Even a relatively minor injury to your foot could lead to an infected open sore (foot ulcer) that's difficult to heal.
Calluses
Corns and calluses develop due to improper friction or pressure, such as when skin rubs against skin ( often happens between adjacent toes) or when shoes or socks rub against a toe or some other part of the foot.
Fungus
Athlete's foot, referred to as tinea pedis by podiatrists, is a common fungal infection of the skin of your feet. The fungus, tinea pedis, is caused by dermatophyte and is contracted from public environments. The fungus then grows in the warm and moist environments such as shoes, socks, locker room floors, public swimming pools, and pedicure tubs. It can be difficult to eradicate.
Psoriasis
Many cases of pediatric onychomychosis (children's fungal toenails) involve an underlying nail problem which results in a nail deformity. This in turn makes the toe nail susceptible to fungal nail infections. Some of these nail problems include conditions such as psoriasis and ectodermal dysplasia, or congenital problems, among others. Another common reason for infections is nail trauma that caused subungual hematoma and damage to the nail matrix. If a child has chronic ingrown nails, this can also cause deformity of nail which in turn can cause fungal infections.

Achilles Tendonitis
Because the heel's growth plate is sensitive to repeated running and pounding on hard surfaces, pediatric heel pain often reflects overuse. Children and adolescents involved in soccer, track, or basketball are especially vulnerable. One common overuse syndrome is Achilles tendonitis. This inflammation of the tendon usually occurs in children over the age of 14. Another overuse syndrome is plantar fasciitis, which is an inflammation of the band of tissue (the plantar fascia) that runs along the bottom of the foot from the heel to the toes.
Ankle Sprain
A fracture of the 5th metatarsal base (located at the outside edge of the middle of the foot) occurs commonly after the foot is injured by turning inward or is crushed. This fracture is sometimes called a dancer's fracture. The outside edge of the foot becomes tender, and a swollen bruise develops. The cause and symptoms may be similar to those of a sprained ankle. A cast is not usually necessary but can make walking easier. Crutches and a protective walking shoe may be needed for a few days. These fractures heal relatively quickly. Fractures of the shaft of the 5th metatarsal bone (Jones fractures) are less common than dancer's fractures and do not heal as easily.

Before explaining bunion removal surgery, we should first explain what a bunion is. A bunion is a bone at the base of the big toe, also called the metatarsophalangeal (MTP) joint. It forms when the bone or tissue at the big toe joint moves out of place. This forces the toe to bend towards other toes, causing an often painful lump. Bunions are a progressive deformity which means that it worsens over the years producing the characteristic bump that becomes increasingly prominent.

Hammertoes
Also, fat and muscle tissue on the bottom of the feet thin with aging; due to this pointy outgrowths of bone (spurs) and bumpy outgrowths of bone are more likely to press against the skin. When friction or pressure occurs regularly in the same area, a corn or callus forms gradually. Problems such as hammertoes frequently have corns associated with them, since the tops of the affected toes tend to be pressed against the tops of the shoes' toe boxes. In addition, poorly-fitted socks that create irritation can lead to the development of corns.
