Rheumatoid arthritis (RA) is an inflammatory type of arthritis that can destroy the joint cartilage. RA can occur at any age. RA generally affects both knees.

For patients with more advanced arthritis, use of a cane opposite the affected hip can help transfer weight away from the painful hip and improve walking ability. A walker can also be used. Arthritis, however, is progressive. Even with treatment, it will worsen over time. Weight loss can help decrease stresses on all of the joints.
Primary Care
Several things must be done to be prepared for surgery. You will need medical clearance from your primary care doctor. If you have any cardiac history, clearance from your cardiologist may be required as well. You must inform us of any medical problems and provide us with a list of your current medications and supplements. You will also be required to have a pre-admission appointment at the hospital. Being as â€œhealthy as possibleâ€ prior to surgery is another good idea. Eating a balanced diet of nutritional food and being at an optimal weight is important. Antioxidant scanning and supplement recommendation is offered in our office as well. Low antioxidant scores have been linked to poor incision healing after surgery. Stop all aspirin and anti-inflammatory medications as well as all supplements 14 days before surgery. These medications, vitamins, and supplements are associated with excess bleeding. If you sustain scratches or scabbing on operative leg, your surgery will likely be cancelled. Avoid yard work, ant mounds (bites), and wear long pants after dark to prevent mosquito bites. Also keep small animals off your lap as their claws can scratch you. Any skin abnormality increases the risk of infection. Instructions for skin preparation, pre-surgery medication protocol, and medication/supplement discontinuation will be given to you. It has been recommended that you start Miralax the day prior to surgery and continue taking it after surgery until your bowel movements return to normal. Pain medications tend to cause constipation. Finally, it has also been recommended that you change your bed linens, towels, and wear clean pajamas the day prior to surgery.

Because the goal of PRP therapy is to resolve pain through healing, it could prove to have lasting results. Initial improvement may be seen within a few weeks, gradually increasing as the healing progresses. Research studies and clinical practice have shown PRP therapy to be very effective at relieving pain and returning patients to their normal lives. Both ultrasound and MRI images have shown definitive tissue repair after PRP therapy, confirming the healing process. The need for surgery can also be greatly reduced by treating injured tissues before the damage progresses and the condition is irreversible. MRI
The diagnosis can be made with a clinical examination and if necessary confirmed with an MRI scan, which is approximately 95% accurate for detecting meniscal tears. Because the meniscus does not heal, the damaged portion needs to be removed. The smaller the tear present the less amount of tissue needs to be removed and conversely, with a larger tear, more meniscal tissue must be removed. Obviously, because it is an important transmitter of stress in the joint, it is advantageous to treat a meniscus tear when it is small so that less tissue has to be removed. Certain tears that occur where the meniscus connects to the joint can be repaired by sewing. These tears usually involve the entire meniscus and occur in younger individuals. Removal of the entire meniscus in these large tears predisposes one to significant arthritis, therefore preservation of the meniscus is extremely important. Because the meniscus has no real blood supply we rely on the blood supply of the neighboring tissues to grow into the tear and heal it. Accordingly, the tear must be less than 5mm or closer to the joint lining for any meniscal tear to predictably heal. Special instrumentation is required for meniscal suturing. Large needles with sutures are passed up through the joints and exit the skin on the side of the meniscus that is being repaired in order to retrieve the sutures.

Arthroscopy
Open surgery may be necessary and, in some cases, may be associated with better results than arthroscopy. Open surgery often can be done through small incisions of just a few inches.
Hip Replacement
Total hip replacement is a common orthopedic procedure. As the population ages, it is expected to become even more common. Hip replacement surgery involves removing the head of the thighbone (femur) and replacing the ball-and-socket mechanism of the hip with artificial implants. This relieves pain and improves mobility.
Knee Replacement
Total knee replacement surgery is a procedure in which a painful or poorly functional knee is replaced with artificial components. Dr. Mead will recommend this surgery if your knee is highly damaged by arthritis or injury. Most total knee replacement take approximately 2 hours.
Knee Arthroscopy
Knee arthroscopy is used in diagnosis and treatment of a number of knee disorders and injuries. One common use is in the treatment of meniscal tears. Injuries or arthritis can cause the meniscus to become torn, resulting in knee pain, swelling and diminish range of motion. Dr. Mead will perform a knee arthroscopy to remove the damaged portion of the knee.

ÂAntiÂ-inflammatory medications are the most common treatment for arthritis. They all work by inhibiting the formation of cyclooxygenase 1 and 2 which are precursor molecules in the inflammatory cascade. These medications do not alter the progressive deterioration that is the hallmark of osteoarthritis, but can help with some of the symptoms of swelling, warmth, and pain. As the arthritis progresses, these medications tend to help less and less. I liken them to cold medication, Âcold medications can make you feel some better, not great, but cannot shorten the duration of the cold. A significant problem with these medications is that heart attack and stroke are associated with their use and can occur early in the treatment course. The risk of this remains relatively low, however. Also, many people do not tolerate these medications because of stomach upset, and ulcer formation is a concern.

Physical Therapy
The second group is the Bandaid group. This includes all traditional forms of treatment and includes bracing, cortisone and gel injections, antiÂ-inflammatory medications, and Physical Therapy. All of these can help with the symptoms of arthritis, but do nothing to alter the progressive deterioration of the joint. Think of cold tablets when you have a cold. They help with some of the symptoms, but donâ€™t make you feel great and certainly donâ€™t shorten the disease. As opposed to holistic options, these have all been proven in a multitude of well designed studies to help control the symptoms of arthritis. Implicit in their use is that one is merely waiting until the symptoms worsen and then either live with significant limitation, or undergo replacement surgery. In general, these options can provide moderate, but not excellent relief of symptoms. Unlike the holistic group, there are documented potential damaging side effects with most of these options. This doesnâ€™t mean they are less safe than holistic options. It means they have undergone the intense study and scrutiny that none of the holistic options have.
