Heel pain is the fourth most common reason for patients to visit their family doctor's office. The most common cause of heel pain is plantar fasciitis. There are other causes of heel pain such as rheumatoid arthritis and gout but plantar fasciitis probably makes up about 95 percent of the causes of heel pain.
Plantar Fasciitis
The term “tendinitis” refers to an inflamed tendon and the term “fasciitis” refers to an inflamed ligament, often the plantar fascia which is the large ligament that supports the foot. Inflammatory conditions often respond to anti-inflammatory medications, ice and massage and tend to be self limiting. There are cases in which conventional treatments for tendonitis and fasciitis persist. Cases of plantar fasciitis that persisted despite treatment became known as “intractable” plantar fasciitis. Surgical treatments were often utilized in that scenario.

Work Related Diseases: A work related injury occurs after a defined injury occurs at a specific place and time. A work related disease is something that occurs due to the work environment but may occur gradually over time. An example of this in the hand is carpal tunnel syndrome which is a repetitive motion or repetitive strain injury. An example of this in the foot is plantar fasciitis, a condition in which the ligament that supports the foot becomes damaged or inflamed.

Ultrasound
The fascia or diseased tendon then remodels or rebuilds over a period of 12 weeks. This process can be observed under diagnostic ultrasound. ESWT is not a symptomatic treatment although the area treated can feel better in three weeks. It is a curative treatment, and the cure can be demonstrated to occur better than 85% of the time via ultrasound.

A tarsal tunnel syndrome test often includes an NCV, nerve conduction velocity test. If tarsal tunnel syndrome is found, an MRI may be ordered to look for the existence of a growth which may be pressing up against the nerve.

Bunion surgery may be performed with IV sedation and a local anesthetic which is administered by an anesthesiologist. General anesthesia may also be utilized at times. A long acting local anesthetic is injected by the surgeon before the surgery is over so that there is no or minimal pain immediately after the surgery, so you will likely not feel much pain.

Proprioceptive exercises. The term “proprioception” refers to one's ability to perceive the position of body parts at any given time. For example, if the ankle is about to turn, the body senses this and automatically starts contracting the peroneal muscles to prevent a sprain. The faster the body does this, the more likely the ankle sprain will be prevented. Proprioception exercises are designed to “speed” the body's ability to respond to motion that can cause injury.

Treatment of chronic exertional compartment syndrome is generally conservative focusing on changes in training, shoe gear, manual therapy, orthotics and other biomechanical interventions. Surgical treatment may be needed if there is risk of muscle or nerve damage and involves a fasciotomy, cutting the fascia to release pressure or fasciectomy, in which a section of fascia is removed.

Over time, the movement of the big toe angles in toward the other toes, sometimes overlapping a third toe, known as Hallux Valgus. The growing enlargement or protuberance then causes more irritation or inflammation. In some cases, the big toe moves toward the second toe and rotates or twists, which is known as Hallux Abducto Valgus. Bunions can also lead to other toe deformities, such as hammertoe.

That may be caused by subtalar joint overpronation, midtarsal joint oversupination, functional equinus or poor shoe design. Sonographic examination often demonstrates thickening of the fascia, scar tissue, and sometimes calcification of the fascia. Calcification of the fascia used to be termed "heel spur" syndrome. X-rays are two dimensional representations of the problem leading to that term. The underlying mechanical problems need to be addressed at this stage.
