What do primary care doctors say about Phentermine? Oh man, I’ve heard a lot of it. The one thing I’d always chuckle at is when they say Phentermine is just legalized speed. Okay doc, well first off it’s not speed. Speed’s an amphetamine. Anybody that’s taking amphetamine, go have them do a drug screen. It will come back negative. Phentermine is not an amphetamine, so quit calling it legalized speed because it is not. It is a matter of fact less like speed than Pseudofedrine is. So if you’re giving them Claritin D, Allegra D, Actifed, Pseudofed, Zyrtec D, guess what? You’re kind of giving them legalized speed because you can actually make speed out of that stuff. Cannot make an amphetamine out of Phentermine. Phentermine is a non-convertible reaction. Just ask the druggies, they don’t even steal when they break into the pharmacies. They just leave that stuff alone. They don’t even want that stuff. Non-addicting, non-amphetamine. Cannot make it an amphetamine. Now, they’re going to clear out the Actifed Pseudofed rack, okay, because you’ve been giving them that, okay, because that’s actually more like an amphetamine than Phentermine is. So you’re the one giving them legalized speed, not me. I don’t give amphetamines for weight loss. So please stop calling it. Now what is Phentermine? Is it a stimulant? Yes. Is caffeine a stimulant? Yes. Is Pseudofedrine a stimulant? Well, half the people say it is. The other half, no. When I take Pseudofedrine, kind of makes me drowsy. Other people get kind of wired up with it. Is amphetamine a stimulant? Yes. Okay, well, where do we give amphetamines to? Treatment of ADD and narcolepsy. Well, let’s skip the narcolepsy one. Let’s go straight to the treatment of Attention Deficit Disorder. An area that comes in under different seminars, you know, ADD, ADHD, whatever, but it’s still a treatment for attention deficit and we still use it. And you know what’s interesting about this? Because Phentermine gets a lot of bad press and you know, other physicians, they say it’s legalized speed. Let me ask you a question, Doc. Why are you giving it to children? You’ve been doing this for, I don’t know, 15 years of their lives. You’ve been giving amphetamines to children for treatment of their ADHD and their ADD and you’re worried about Phentermine? Aren’t you worried about the long-term side effects from giving a child amphetamines for 15 years? Is that thought not occurred to you? Well, it probably hasn’t. The reason why it shouldn’t bother you too much because there is none. Amphetamines, Phentermine, pseudoephedrine have been legal in the United States since 1959. So, I’d say the majority of people that started taking it back then, well, they’re no longer with us and they probably all died because their heart stops. But it wasn’t because of Phentermine. Now, they, people will say Phentermine gives me the shakes. No, it does not. It does not give you the shakes. Hypog

People that gain a lot of weight, they are always hot. Okay, they always feel hot. You will see skinny people that are always cold. Okay, and people like to say, well, that’s because they got all this insulation on. It is not insulation, okay? That is not the determining factor. It’s surface area to weight ratio determines that. People that are real skinny are really cold all the time. They’ll be in a 70 degree room complaining about how cold it is. The same person that weighs 300 pounds would be like, Jesus, can you kick on the air conditioner? They’re comfortable at 65, 60 degrees, but it’s the surface area. Thin people are more like radiators. They have a lot of surface area and are able to give off heat very well. Larger people are more like balls, very inefficient radiators. Where this comes into a medical condition that a lot of us have seen is in children with febrile seizures. So the child gets sick with a flu, a cold, sore throat, could be strep throat, but just a simple virus even. And they’re running along at the house with a temperature of 100, 101. Mom’s getting a little bit nervous. All of a sudden, they spike a temperature to 103, 104 degrees and have a seizure. Mom freaks out, comes running to the emergency. Oh my God, oh my God, my baby’s having a seizure. Well, no, the temperature is 104 degrees, 105 degrees. We will all have a seizure at 104, 105 degrees. Every human out there will have a seizure at 105 degrees. Your body cannot take that kind of temperature. Well, what’s the difference between us and the child? The child is a ball, small arms, small legs, small limbs, very, very inefficient radiator. When they grow up, they grow out of it. And the reason why is now I have some areas to get rid of heat. This is my radiator, but that’s what that is. So when you talk about weighing 600 pounds, trust me, your thermal energy, in other words, your ability to get rid of heat is almost zero. And so that loss in a human on a daily basis, that can almost be 90% of your calories burning off in heat, or in their particular case, less than 1% as far as heat. And no, they cannot move. There is no physical activity. The only physical activity they’re getting is their hearts beating and their lungs are breathing. That is it. Their energy requirements to maintain their body at 600 pounds, probably 300 a day would do it. Almost nothing. And that brings up an interesting point. We mentioned this earlier, and it’s a really interesting study, and it was done in 1976. And it’s in the Guinness Book of World Records. How long can a human body go without eating? I’ve asked this question a lot to my patients over the years, and I always chuckle at their responses. They’ll say, oh, seven days. I’m like, no, no, no, no, that’s water. Human body can go for seven, 10 days, maybe two weeks without water. But how long can we actually go without food? But it is in the Guinness Book of World Records, 1976. It was an interestin

So when we’re looking at motivation, how do we get people motivated to lose weight? We first have to understand why they’re here to lose the weight. And it’s been my experience, people under the age of 35, 30, the majority of them just want to lose weight to look good at the beach. And so we have a boom season in the spring, you know, where we see a lot of people, oh, I just want to shed 20 pounds, lose weight. I’m like, okay, you know, we can, we can do that. I wish you’d lose weight and never see me again for the rest of your life. But I understand you want to lose 20 pounds, go to the beach and say, Oh, hop, sing, bring me another Ma Tai, please. You know, and just lay on a beach and drink and eat and have a good time. It’s okay. Those, that type of people, they’re there, they’re always going to be there. And that’s how people are. Other people I’ve noticed over the age of 35 to 40, they’re not so much there to look better. They want to feel better. So we’re talking about knee pain, breathing, sleeping, moving. Then you get the more elderly population that comes and sees me to lose weight. We have a big problem. I just don’t have any giddy up. I have no energy. I want to be able to spend more time with my grandkids. I want to go to the park and I can’t, I’m afraid if I fall down, I won’t be able to get up. I don’t want to see them breathing so hard. You know, I don’t want to have a heart attack when I’m out with them. So I think as age goes along, we have different motivating factors as to why we want to lose weight. But the end all goal is still the same to lose the weight and hopefully understand what it is we’re doing wrong and never put it back on. And in order to need that, to do that, we need knowledge. We need truth. We need to know the truth about weight. This country spends $4 billion a year on advertising for weight loss. Now I don’t care who it is. It could be Jenny Craig, any kind of thing. Golos is a big one on right now. None of these things are truth. Okay. They’re all wanting to sell you a meal plan, medicine. I’m not here wanting to do any of that. I want to talk about the truth of weight because you don’t need me to lose weight. You might need my help to lose weight, but you don’t need me. Only thing you need to lose weight is knowledge. The motivational factor side to it, I can promise you, you’re going to feel good. Your knees will feel better. I can’t honestly tell you how many people I’ve come to see me that complained about their knees and that they needed to get the weight off so they can go have a knee replacement. Well, lo and behold, they lose a hundred pounds, no more knee pain. Don’t need a knee replacement now. Oh, imagine that, you know, we should have taken the hundred pounds off 20 years ago and you wouldn’t have this problem right now, but that’s water over the dam. Yeah, we haven’t really even discussed this yet. Why do insurance companies not pay for people to lose weight? You would think they would. It’s the right thing to do. I mean, these people don’t understand why they’re doing the things they’re doing. They just know they’re gaining weight, but their insurance company, they won’t pay for a visit to my office. None of them hardly ever will. They’ll fight tooth and nail to pay for bariatric surgery. But the question becomes why? It would seem to almost make sense to the average lay person that paying for this would stave off a bunch of problems. And that’s true. The problem is when do these problems occur? Okay, so let’s walk this through the medical economics of our society. Well, we live to be roughly 77 years of age. And when you look at economics of healthcare, two-thirds of our healthcare dollars are spent in the last two to three months of life. So I’m 77 years old. I got two, three months to live. All, pretty much two-thirds of my healthcare dollars are going to be spent right there. I’m going to have a heart attack, wind up in a nursing home, have a stroke, need home health care, get cancer, need hospice. But this is where all the healthcare dollars are concentrated, two-thirds. So, if we knew when people were going to die, the exact date had little digital signatures on their wrist and we wanted to save, gosh, $700 billion a year. I say we start killing these people three months before they’re supposed to die. Well, we’ll save a lot of money doing that. But it stops, stands to reason then, well, let’s look at when your problems are occurring from chronic diabetes. I’ve lost my kidneys. Now I’m on dialysis. I’ve had high blood pressure because I’m overweight. Now I have heart attacks and strokes. Oh, I’ve got to have my knees replaced. Okay. And I’m in my seventies then that’s because I’ve been overweight all my life. And now my knees are breaking down. But none of these problems really happened before the age of 65. Well, before the age of 65, guess what? You’re not on Medicare. After 65, you are. And these insurance companies know this. You are no longer going to be their problem. You’re going to be a Medicare problem. And if you think the federal government’s going to pay for weight loss, they don’t have any money left. It’s over. So no. Do I think the insurance companies should pay for people to lose weight? Absolutely. The federal government should step in and demand this. And the reason why they should demand it is they’re going to be saving money on the long term to where it’s not going to cost them money when they turn, get to be age 77, 78. They’re going to be dying of old age. Great. There was no healthcare expenditure. Why can they not see this? And the reason why? That insurance lobby in Washington, DC.
