Primary Care
Colorectal cancer (CRC) remains the second leading cause of cancer related deaths in Texas. Despite the available evidence for the benefits of screening with stool- based or structural examinations, Texas ranks 48th among states for CRC screening completion. Available data suggests that population-based programs can be particularly effective and that critical components for increasing screening rates include high level champions, stakeholder involvement, a convening entity, data driven approaches, defined goals, understanding assets and resources, creating synergies across systems, and collaborations across public health, community organizations and health care organizations. We propose a center that is informed by the data, addresses all the expectations described in the RFA, and leverages the experiences and expertise of collaborators in relevant disciplines cross the cancer continuum, collegial statewide networks, and shared understanding of current needs across the state. The overall purpose of the Coordinating Center for Colorectal Cancer Screening across Texas (CONNECT) is to galvanize and support CRC prevention efforts in Texas. The Center will be led by Drs. Shokar and Pignone who between them have four decades of experience as primary care physicians, public health scientists and as national and internationally recognized experts and leaders in CRC screening. Between them they have led or co-led 17 successful CPRIT funded cancer prevention programs and have supported adoption and implementation of evidence-based CRC screening strategies across Texas, nationally and internationally. The Center will serve as a convening entity and central hub of resources, tools, and content expertise accessible by stakeholders across the state. The center will consist of: 1) A steering committee that is advisory to the leadership and comprised of physicians in disciplines across the cancer continuum, advocacy groups, CRC screening experts, scientists, leaders of public health and clinic systems and patients representing all geographic regions and populations; 2): Five Cores i. Administrative; ii. Community-based Implementation, Engagement, Education and Health Communication; iii. Clinical Implementation; iv. Modeling, Mapping, Cost effectiveness and Data; v. Advocacy; and 3). A statewide stakeholder network that will include, public, private, governmental, and non-profit organizations, public health organizations, safety net systems, academic health centers, social services organizations, Community Health Worker organizations, payors, governmental entities, advocacy groups, and individuals among others. The work of the center will culminate in creating the infrastructure, capacity and resources to propel Texas' CRC screening efforts forwards. At the same meeting, CPRIT approved a $2.5 million grant to continue and expanded the “Salud en Mis Manos” prevention project ( PP230038 ) to Starr County. This project, led by The University of Texas Health Science Center at Houston, has provided breast and cervical screenings and HPV vaccinations for underserved Latinas in Hidalgo and Willacy Counties since 2019.

David A. Cummings, M.D. is a practicing medical oncologist from San Angelo, Texas. His career in cancer medicine has spanned over 20 years primarily in West Texas. He has led or served on hospital committees including Cancer Committee, Quality Management Committee, Pharmacy & Therapeutics Committee, and Credentialing Committees of several West Texas health institutions. He is currently a community medical oncologist at the Shannon Clinic in San Angelo. He is Board Certified in Medical Oncology by the American Board of Internal Medicine (ABIM). He is a member of the Texas Medical Association and past president of the Concho Valley County Medical Society. Dr. Cummings is a member of the American Society of Clinical Oncology (ASCO) and the American Society of Hematology (ASH). His community involvements include appointment to the City of San Angelo Development Corporation (COSADC), a past president of the board of the San Angelo Symphony and past president of the Texas Southwest Council of the Boy Scouts of America. Dr. Cummings received his bachelor’s degree from Baylor University, medical degree from Texas Tech University Health Science Center School of Medicine and completed residency and fellowship training at Scott & White/Texas A&M Health Sciences Center. Having been initially appointed by Governor Abbott in 2018, Dr. Cummings is currently serving his second term on the CPRIT Oversight Committee.

FDA-approved IO drugs that harness the power of the body’s immune system to fight cancer have made rapid advances in treating patients with cancers who previously had very few options. This includes patients with melanoma, non-small cell lung cancer, kidney and bladder cancer and several others. However, as many as 70% of these patients develop resistance and their cancer progresses, and they are again without options.

Ovarian Cancer
To date, Ruga has conducted several preclinical proof-of-principle studies and established compelling data demonstrating Ruga-S6’s efficacy and tolerability in multiple in vitro studies and in vivo models of cancer. In preclinical models of AML, including patient-derived leukemic cells from FLT3-ITD(+) patients, treatment with Ruga-S6 has been associated with remarkable anti-tumor effects coupled with exceptional tolerability, making Ruga-S6 a potentially ideal drug for AML patients. In preclinical models of advanced, drug resistant ovarian cancer, the treatment with Ruga-S6 led to greater than 95% of animals becoming disease free without any tolerability issues. Preclinical studies in breast, lung, renal and pancreatic cancer models generated equally compelling data.

The De Casa en Casa (De Casa) program was created to specifically address barriers to cervical cancer screening among uninsured, underinsured, those in rural and border counties, and racial/ethnic minorities. The program will continue to serve El Paso County and 60 counties in West and South Texas. Increasing Breast,Cervical,and Colon Cancer Screening and increasing HPV vaccination among Underserved Texans: A collaboration with the United Way's 211 Program
Prostate Cancer
Magnetically Activated and Guided Isotope Separation ('MAGIS") technology will enable production of the stable isotope Ytterbium-176 (176Yb) needed to make the radio-isotope Lutetium-177 (177Lu). 177Lu is an effective beta-therapy agent approved for certain neuroendocrine cancers and soon to be approved for prostate cancer, the second leading cause of cancer death in men, with clinical trials underway for a range of cancers. 177Lu can be used to target small tumors and dispersed, inoperable metastatic cancer using precise delivery molecules. 176Yb is currently only available in small quantities from Russia and that supply is uncertain due to geopolitics and competition for limited production capacity. We have already produced small quantities of medical-grade (99.5%) 176Yb and have secured a funded distribution partnership with Eckert & Ziegler. Development of this isotope production capability will also enable future production of diagnostic isotopes and of other beta and alpha emitting radioisotopes, effective in targeting cancer cells with minimal damage to adjacent healthy cells. Early access to these isotopes will put cancer treatment and medical research institutes in Texas at the forefront of these therapies with opportunities for development and trials of a range of related co?products. MAGIS offers a scalable, reliable, domestic source of 176Yb for 177Lu production to make 177Lu-based therapies readily available in Texas and to the global medical community.
Cancer Care
There are over a million cancer survivors in the state of Texas. It is estimated that one in three women and one in two men will be diagnosed with cancer in their lifetime, and the population of cancer survivors is expected to grow as survival rates improve and the population ages. Decreases in physical functioning after a cancer diagnosis are common and exceed what would be expected due to normal aging processes. Cancer survivors are also at increased risk for developing second cancers; between 15 and 20% of cancer diagnoses occur in people who have had a previous diagnosis. These functional deficits and increased risks of second cancers necessitate accessible and effective programs to help survivors improve their health and quality of life. Physical activity has been shown to improve quality of life and disease-free survival in people diagnosed with cancer, but programs to support survivors in being more active are not widely available or accessible. Overall project strategy: The goal of this dissemination project is to promote broader, sustainable adoption and implementation of the Active Living After Cancer (ALAC) program, an evidence-based intervention to improve cancer survivors' physical functioning and quality of life through increasing their physical activity. The ALAC program has been funded by CPRIT for the past 9 years and is currently offered to cancer survivors and their caregivers through partnerships with community organizations. With an implementation focus on minority and medically underserved populations, ALAC is available in both English and Spanish, and aims to teach cognitive and behavioral skills for increasing physical activity, provide opportunities to engage in different types of light to moderate physical activity, and deliver education content about survivorship concerns and services. It is delivered in 12 weekly sessions, offered either in-person or virtually. We propose to disseminate ALAC to community cancer centers in Texas, partnering with the Texas Society of Clinical Oncology (TxSCO; state affiliate of the Association of Community Cancer Centers), which will provide feedback on our dissemination process and promote the program to its members. We are focusing on community cancer centers because over 85% of cancer patients receive cancer care in the community, and thus these centers serve a broad cross-section of survivors. Specific goals: In order to effectively disseminate ALAC, we will develop an online implementation toolkit (Goal 1), which will include curriculum facilitator guide and PowerPoint presentations; and implementation guide that includes information on screening participants, developing a safety plan, and how to adapt the program to participant needs; participant recruitment materials; and program evaluation guide. We will also develop online training and a standardized Project ECHO curriculum for providing ongoing telementoring and technical assistance to implementers. These products will be shape

Tobacco use is causally linked to ~17 different cancers and a variety of other diseases. Although tobacco use rates have declined in the population overall, this has not been the case within sexual and gender minority groups, which are a rapidly rising demographic in the US. High tobacco use rates, up to 37.7% in some subgroups (individuals identifying as transgender), have led to recognition of LGBTQ+ individuals as a priority population for tobacco control and intervention. Despite their focus on unique stressors and health risk factors for LGBTQ+ patients/clients, LGBTQ+ centers (which provide support services, ranging from social support groups to comprehensive medical care, to primarily LGBTQ+ persons) rarely provide tobacco use disorder care. In PP230003, we propose to disseminate and implement Taking Texas Tobacco Free (TTTF)-a successful, evidence-based tobacco control intervention for cancer prevention that provides tobacco-free workplace policy guidance to de-normalize tobacco use, education and specialized training to care providers, and resources-to LGBTQ+ centers to bolster their ability to provide cessation and relapse prevention interventions to their patients/clients. TTTF has evolved across 6 CPRIT funded awards in behavioral health care and community agency settings and is currently being implemented in medical settings (FQHCs) and homeless-serving centers. Responsiveness to CPRIT priorities include that it: 1) prioritizes populations disproportionately affected by cancer; 2) is an interdisciplinary project; 3) is a collaborative project between nonprofit entities; 4) is a project to eliminate tobacco use to reduce new cases and deaths from tobacco-related cancers; and 5) develops or strengthens the infrastructure supporting the delivery of the most appropriate cancer prevention services. Overall Project Strategy. The previously implemented program, TTTF, will be fully scaled-out for application in centers serving the LGBTQ+ community. TTTF implementation in >/=3 LGBTQ+ centers (final # as time/budget allows) will include the: 1) initiation (or refreshing) of tobacco-free workplace policies; 2) provision of education and specialized training with a focus on how to facilitate trauma-informed, community-tailored (e.g., for individuals undergoing gender-affirming hormone replacement therapy; salient stressors for tobacco use, such as discrimination and internalized stigma), tobacco use cessation and relapse prevention with patients via behavioral intervention and medicinal aids while enabling capacity for a sustainable center-led training program; 3) provision of resources to educate and motivate patients and providers for tobacco use disorder care provision; and 4) engagement and education of the community around these topics. High-Intensity Focused Ultrasound (HIFU) is a form of image-guided therapy capable of noninvasive tissue ablation and drug delivery. Similar to the concentration of light energy to a focal point using a lens, HIFU can concentrate acoustic energy within the body to a region of a few millimeters in dimension. The energy at the focus is sufficient to thermally coagulate tissues—rendering them permanently destroy—without causing any damage to intervening tissues. Because HIFU is noninvasive and does not involve ionizing radiation, it is very well-suited for use in pediatric medicine.

Robotic Surgery
Because the complications of prostatectomy (incontinence, erectile dysfunction) are common and devastating regardless of technique used (manual or robotic surgery), it is clear that prostate cancer is being dramatically over-treated. There is growing evidence that patients with low-risk disease may benefit significantly from a minimally invasive focal therapy (treatment of only the cancerous tumor) and avoid complications associated with prostatectomy and other “whole gland” therapies such as radiation.

Skin Cancer
7 Hills Pharma is a Texas-based company developing novel therapies for cancer. Our drug, 7HP349, could benefit melanoma and other solid tumor patients resistant to immune checkpoint inhibitor (ICI) therapy. ICIs use the immune system to fight tumors, and have made rapid strides in treating melanoma, non-small cell lung, renal, bladder and other cancers. However, 43% of melanoma patients develop ICI drug resistance and are left with limited treatment options. 7HP349 addresses a primary cause of ICI resistance, the inability of T cells to infiltrate tumors, and has shown the ability in preclinical studies to reverse ICI drug resistance and reduce or eliminate tumors. This proposal aligns with CPRIT's mission to bring accessible treatment options to cancer patients, in particular to those in Texas. We have Orphan Drug Designation, Fast Track status, and an active IND for this indication with the FDA and have completed a Phase I clinical trial to show that 7HP349 is safe and well tolerated in humans. CPRIT funds will enable a Phase Ia/IIb study to confirm drug safety and positive responses with 7HP349 in ICI-resistant melanoma patients with progressive disease. If successful, this study will represent a new treatment paradigm for ICI-resistant cancers. 7 Hills will expand operations in Houston, creating high-quality jobs in drug development and clinical operations. 7 Hills is committed to Texas and will ensure that CPRIT funds are spent in Texas, whenever possible.
