Carpal Tunnel Syndrome
Carpal tunnel release surgery may be indicated when non-surgical treatments for carpal tunnel syndrome fail. It may also be performed when hand muscles lose strength or become smaller due to impingement of the median nerve, which runs through a small tunnel in the wrist. Swelling or other conditions can cause the nerve to become pinched, leading to pain and an inability to properly use part of the hand. Carpal tunnel release involves a hand incision to open a ligament pinching the median nerve. Doing this relieves the pain, numbness and tingling for many patients.

Physiatrists also treat serious disorders of the musculoskeletal system that result in severe functional limitations. They would treat a baby with a birth defect, someone in a bad car accident, or an elderly person with a broken hip. Physiatrists coordinate the long-term rehabilitation process for patients with spinal cord injuries, cancer, stroke or other neurological disorders, brain injuries, amputations, and multiple sclerosis.

Exams vary from patient to patient, but often a routine x-ray is used for most cases of back or neck pain. If you are suffering from severe or continuous pain, or pain associated with numbness or weakness in the limbs, you may require an MRI scan. Some cases may require a CT to get a better picture of the changes in the spine. Bone scans are often done in cases when osteoporosis or tumors of the spine are suspected.

Traumatic brain injuries are classified as mild, moderate, or severe depending upon impact on level of consciousness at the time they occur. These result from the brain violently coming into contact with the skull or the skull being penetrated. Following the initial event, additional factors such as other associated injuries can cause oxygen delivery to the brain to be compromised, such as when blood pressure falls, causing the brain to be further deprived of needed nutrients. Symptoms of a mild injury, often called a concussion, include brief or no loss of consciousness, confusion, sleep problems, sensitivity to light, headache, vomiting, poor balance, fatigue, and blurry vision. Symptoms of moderate to severe brain injury include a more significant impact on level of consciousness, or dilated pupils, restlessness, slurred speech, and severe confusion. Symptoms related to a brain injury may show up immediately or not be recognizable for days or weeks. Seizures may occur. Diagnosis is typically made based on a history of events, neurological examination, and CT scan.
Chemotherapy
Although surgery doesn’t cure the disease, it can lengthen the time of survival, increase the quality of life, and reduce intracranial pressure (pressure inside the skull that affects brain function). Unfortunately, it is usually impossible to completely remove the tumor and new tumors typically arise in the tumor bed regardless of the amount of tumor removed. New tumors may arise in other locations as well. Surgery is often followed by a course of radiation and chemotherapy. Steroids may be used to reduce swelling around the tumor during recovery and treatment.

Occipital neuralgia is a condition in which the sufferer develops throbbing or piercing headaches. The headache may feel as though it begins in the neck and spreads to the head. More commonly, it is felt on one side of the head, although it may be felt on both sides. This condition is more common in women and is often thought to be due to the occipital nerve that runs from the spinal cord to the head becoming trapped in muscle. In addition to painful headaches, symptoms include sensitivity to light and a tender scalp. Causes include spinal cord compression, arthritis in the neck, tumors, diabetes, and injury, such as whiplash. Treatments include anti-inflammatory medication, muscle relaxants, pain medication, nerve blocks, steroid injections, and physical therapy. Severe cases may require surgery.

Physical Therapy
Laminectomy is typically performed when more conservative therapy, such as medication or physical therapy, fails to relieve symptoms. Decompressive lumbar laminectomy (DLL) is performed when the sides of the spinal canal are narrowed, pinching the spinal nerves, while MLD (microlumbar discectomy) is performed when a disc (or discs) are herniated.
Occupational Therapy
The ulnar nerve runs from the neck down to the ring and little fingers. It controls some of the motion in the hand and forearm. This nerve can become entrapped in an area called the cubital tunnel, found behind the inner elbow. The nerve can become compressed by fibrous bands of tissue near the bony bump inside the elbow, leading to pain, weakness in the hand, and numbness or tingling of the ring and little fingers. Surgical correction is sometimes needed to free the nerve and the surgeon may decide that ulnar nerve transposition will work best. During this procedure, the ulnar nerve is moved from its current location to a new place in front of the elbow, either on top of, within, or under the muscle. Physical therapy or occupational therapy may be helpful to help restore normal function.
Slipped Disc
A ruptured disc, also called a herniated disc or a slipped disc, may become painful if it causes pressure on a nerve root or the spinal cord. The disc is referred to as being ruptured because the tough outer lining has ruptured or split, allowing some of the inner disc cartilage to be released. The inner disc cartilage, or nucleus, feels like cooked crab meat or shrimp when squished between your fingers. When this material presses against a nearby nerve root or the spinal cord, pain in the back or pain, numbness, or tingling in the body or limb areas served by the nerve may become evident. The sufferer may notice impaired movement in the affected area. In some cases, such as when no nerve root is severely compressed, no symptoms of a ruptured disc are apparent and the patient may be unaware of it until it is found during diagnostic testing. Treatment for a ruptured disc is often dependent on whether or not the patient is experiencing symptoms. Chronic pain or interference with the ability to work or care for oneself frequently necessitate treatment, including physical therapy, a short course of pain medication, or muscle relaxants. Often, a brief rest period is ordered, followed by stretching or strengthening exercises. Occasionally, surgery is required when conservative measures fail or symptoms such as loss of bowel or bladder control are present. Some patients are candidates for minimally invasive spinal surgery to relieve severe symptoms. A herniated disc without symptoms requires no treatment other than following good ergonomic practices and avoiding back strain.
