Dr. Holsopple, is a graduate of the University of Pittsburgh Medical Center a three year surgical residency program. She is board certified by the American Board of Multiple Specialties in Podiatry and Board Certified by the American Board of Foot and Ankle Surgery. She is accomplished in forefoot and rearfoot reconstructive diabetic deformities, ankle arthroscopy, Ilizarov External Fixation and trauma of the foot and ankle.

3. The patient and his or her caregivers are then educated about proper wound care and prevention. They’re taught how to care for wounds at home and receive information about nutrition and exercises that may aid in the healing process and prevent future wounds. For example, foot wound patients are given instruction about protective measures for their feet and walking, and sometimes special shoes that protect the feet and relieve pressure are prescribed.

While underlying causes of chronic wounds vary from patient to patient, they tend to occur in patients with poor blood circulation, diabetes, or for other reasons related to a patient’s condition. Pressure ulcers (sometimes called ‘bed sores’), for example, can develop in nursing home patients who are confined to bed, and can evolve into chronic wounds. Likewise, for a person with diabetes, a simple foot blister may result in a chronic open sore, serious infection, gangrene, and may even require amputation.

The shape of your foot molds the cast. It lets your ulcer heal by distributing weight and relieving pressure. If you have Charcot foot, the cast controls your foot’s movement and supports its contours if you don’t put any weight on it. To use a total contact cast, you need good blood flow in your foot. The cast is changed every week or two until your foot heals. A custom-walking boot is another way to treat your Charcot foot. It supports the foot until all the swelling goes down, which can take as long as a year. You should keep from putting your weight on the Charcot foot. Surgery is considered if your deformity is too severe for a brace or shoe.

Patients are put on a program of relative rest followed by physical therapy for the first 6 weeks. After about 6 to 12 weeks, patients are re-evaluated for improvement. Some patients with more difficult injuries may take more than one treatment to achieve successful outcomes.

According to the American Diabetes Association, about 15.7 million people (5.9 percent of the United States population) have diabetes. Nervous system damage (also called neuropathy) affects about 60 to 70 percent of people with diabetes and is a major complication that may cause diabetics to lose feeling in their feet or hands. Foot problems are a big risk in diabetics. Diabetics must constantly monitor their feet or face severe consequences, including amputation.

The main advantages of custom-made orthotics are the excellent, personalized fit and the individualized correction of biomechanical faults. These factors help ensure rapid adaptation and relief of underlying and associated musculoskeletal complaints.5 Especially when dealing with poor support for the pelvis and spine, a custom fit provides a more predictable response and longer-term symptom relief. The materials and in-shoe fit can be selected to work well in the specific patient’s usual shoes. Activity levels and body mass can also be taken into account when ordering custom orthotics. Specific corrections, asymmetrical posting and lifting, and support for unusual and anomalous anatomy can also be requested. This becomes important when it is recognized that abnormality and asymmetry are common.6 A heel spur correction built into a custom orthotic will often provide immediate relief. Any necessary modifications and further customizing are usually included in the laboratory’s fee arrangements.

People with diabetic neuropathy in their feet may have a false sense of security as to how much at risk their feet actually are. An ulcer under the foot can develop in a couple of hours. The primary goal of therapeutic footwear is to prevent complications, which can include strain, ulcers, calluses, or even amputations for patients with diabetes and poor circulation. In addition to meeting strict guidelines, diabetic shoes must be prescribed by a physician and fit by a qualified individual, such as a certified Podiatrist. The shoes must also be equipped with a removable orthotic. Foot orthotics are devices such as shoe inserts, arch supports, or shoe fillers such as lifts, wedges and heels. The diabetic shoes and custom-molded inserts work together as a preventative system to help diabetics avoid foot injuries and improve mobility.
