Generally, arthritis of the knee affects either the lateral compartment (outside), medial compartment (inside), or the patellofemoral compartment. Occasionally a patient is affected in two or all three compartments.

Medical Weight Loss
Most arthritic conditions do not require surgery. Moreover, the first forms of treatment for arthritis of the knee which most patients should consider are non-invasive conservative treatments. These prove highly effective for most patients and include weight loss and exercise. These plans are among the most successful conservative treatments at the SandersClinic. Often times weight reduction alone can significantly improve the patient’s condition. Dr. Sanders may also prescribe anti-inflammatory medication and nutritional supplements rich in Glycosaminoglycans and Chondroitin Sulphate, as well as Vitamin D. This combined with an individualized program of stretching exercises (to improve flexion and especially extension of the knee) and strengthening exercises (to build quadriceps muscles) can yield satisfactory results.
Primary Care
The age-adjusted rate of lower-extremity amputation (LEA) in the diabetic population is -15 times that of the nondiabetic population. Over 50,000 LEAs were performed on individuals with diabetes in the United States in 1985. Among individuals with diabetes, peripheral neuropathy and peripheral vascular disease (PVD) are major predisposing factors for LEA. Lack of adequate foot care and infection are additional risk factors. Several large clinical centers have experienced a 44-85° reduction in the rate of amputations among individuals with diabetes after the implementation of improved foot-care programs. Programs to reduce amputations among people with diabetes in primary-care settings should identify those at high risk; clinically evaluate individuals to determine specific risk status; ensure appropriate preventive therapy, treatment for foot problems, and follow-up; provide patient education; and, when necessary, refer patients to specialists, including health-care professionals for diagnostic and therapeutic interventions and shoe fitters for proper footwear. Programs should monitor and evaluate their activities and outcomes. Many issues related to the etiology and prevention of LEAs require further research.

When the carpal tunnel surrounding the thumb tendons as they pass from the wrist to the thumb becomes irritated or swells, the patient experiences tendonitis of the wrist. Pain and tenderness are evident when grasping or pinching. This is often caused by overuse and is also associated with some cases of arthritis.

MRI
A MRI is usually ordered to be certain that the tear of the MCL is not at its lower end. Rarely is it, and if so a limited l surgical procedure is necessary to repair it back to the bone. Having said that, in the vast majority of cases, we have found it most efficacious to not interfere with the natural healing potential of those ligaments that can heal on their own. Furthermore, many, but not all, meniscal cartilage tears will also heal without surgical intervention. The MCL, PCL, and many meniscal tears can be treated in this way. We place a cylinder cast or a locked brace on the leg that allows patients to walk without crutches and do straight leg raising exercises for two weeks. After two weeks, a cast gets loose, and we remove it, do some range of motion exercises and place a new one for another two weeks. When a brace is used, it is removed, some range of motion exercises performed, and it is replaced, tightening the Velcro. After that we remove the cast or brace and find that these ligaments are healed.

Patients aged less than twenty years, involved in contact or competitive sports, with X-ray, MRI, or CT scan evidence of bone loss are particularly at risk for failure of arthroscopic labral repair. This group typically includes those with multiple dislocations, and those whose shoulder easily dislocates with the arm below the horizontal. A particularly prone group is those awaken from sleep with their shoulder dislocated.

Arthroscopy
Dr. Sanders’ procedures include an examination under general anesthesia, in which the laxity of the patella to medial and lateral forces is documented, and a diagnostic and/or surgical arthroscopy is performed. Arthroscopy will directly identify and confirm the problem and correct any other intra-articular abnormalities such as torn menisci, a pathologic medial plica (a relatively painful but harmless developmental condition), as well as address chondral surface pathology.
Wound Care
Total Contact Casting (TCC) may also be employed to offload plantar ulcers, which show signs of wound care failure. Once confirmed, a gastrocnemius +/- soleus recession is indicated to correct and restore balance of load on the foot. Distributing the load reduces risk of other chronic foot conditions such as diabetic ulcers. TCC may also be employed to offload plantar ulcers showing signs of wound care failure.
Ankle Surgery
At the Sanders Clinic, all foot and ankle surgery patients are given the benefit of multimodal analgesia. By attacking the pain through several different pathways, fewer narcotics are necessary, and subsequently, the negative side effects of narcotic use can be diminished or eliminated. These medications include an anti-inflammatory such as Meloxicam, a nonnarcotic analgesic such as Tramadol, acetaminophen, also known as Tylenol, and gabapentin – which calms down peripheral nerve pain. These medications are taken around the clock. The need for a Class 3 narcotic analgesic, such as hydrocodone, is drastically diminished, therefore fewer side effects such as nausea and/or constipation occur. Phenergan is prescribed to help with nausea. Taking two Tylenol every four hours is a good alternative.

Bunions
Dr. Sanders specializes in injuries and conditions affecting the foot and ankle - from bunions and ankle sprains to foot fractures and adult acquired flat foot deformity.
Arthroscopy
When surgery is necessary, Dr. Sanders’ vast experience in arthroscopy provides patients with minimally traumatic surgical options – using well proven techniques for repair and reconstruction of the rotator cuff tendon and treatment for the unstable shoulder.
Knee Replacement
There have been a number of advances in total knee replacement surgery over the years. Dr. Sanders utilizes the latest technology with a proven accelerated rehabilitation program in order to achieve optimal results and a more rapid recovery.

Ulcer
Background: Limited ankle dorsiflexion has been implicated as a contributing factor to plantar ulceration of the fore-foot in diabetes mellitus. The purpose of this study was to compare outcomes for patients with diabetes mellitus and a neuropathic plantar ulcer treated with a total-contact cast with and without an Achilles tendon lengthening. Our pri-mary hypothesis was that the Achilles tendon lengthening would lead to a lower rate of ulcer recurrence.

Articular cartilage wraps the end of the bones and has a smooth, slick surface that allows the bones to slide over one another without friction or direct contact – minimizing pressure during movement. While cartilage tissue is void of nerves, worn or torn areas on the cartilage may result in inflammation of the joint and cause pain. Injuries to the articular cartilage often present as tears or holes in the surface of the cartilage. Lesions can vary in thickness of injury. Most superficial injuries, or partial thickness injuries, are not serious. If the lesion is large, full thickness, and exposes the bone below, it could result in arthritis.

Sports Medicine
Fifty years ago, Dr. Hughston, one of the founding fathers of American Sports Medicine, imparted a vernacular term for rotational deformities of the lower extremity – “Miserable Malalignment.”
Physical Therapy
Options range from shoe inserts, orthotics, bracing and physical therapy for elderly and/or inactive patients to reconstructive surgical procedures in those wishing to remain more active. These treatments restore proper function and alignment of the foot by replacing the damaged muscle tendon unit with an undamaged, available and expendable one, lengthening the contracted Achilles tendon and realigning the Os Calcis, or heel bone, while preserving the joints of the hindfoot.
Tennis Elbow
Lateral epicondylitis, or tennis elbow, is a common problem that affects the outside (lateral) part of the elbow and results from degeneration and inflammation of the origin of the extensor carpi radialis brevis (ECRB) tendon. It often occurs when the tendon is repetitively overused or overstressed.

Since the ankle sprain is a condition manifested by instability of the joint, Dr. Sanders performs a careful physical exam on the ankle by palpating and stressing the ligaments, along with the Achilles tendon. Following this, he decides whether or not further testing is necessary. If so, the patient will receive a Telos Stress X-ray. This low-cost exam uses the Telos Stress Device to apply stress to the ligaments of both ankles under X-ray. A comparison between the normal and abnormal ankle X-rays using digital technology will then allow Dr. Sanders to determine if there is a significant difference between the two ankles.

General Podiatry
RESEARCH DESIGN AND METHODS – Patients presenting with new ulcers (duration 1 month) to a dedicated diabetic foot clinic between 1994 and 1998 were studied. Outcomes were determined until March 2000 (or death) from podiatry, hospital, and district registers. Baseline clinical examination was done to classify ulcers as neuropathic, ischemic, or neuroischemic. Five-year amputation and mortality rates were derived from Kaplan-Meier survival analysis curves.
Achilles Tendonitis
Inflammation of this tendon is known as Achilles Tendonitis. It is a degenerative condition of the tendon either in its substance or at its insertion point on the heel. Although is it commonly called tendonitis, there is no evidence of true inflammation. Achilles Tendonosis is probably a more medically correct term. It occurs when the overstressed tendon becomes degenerate causing enlargement of the tendon, restricted flexibility and pain.
Ankle Instability
Ankle sprain is the most common injuries for athletes. Anyone who has played a sport is likely to have experienced this type of injury. Also known as ankle instability, or a lateral ankle ligament tear, it occurs when someone awkwardly plants the foot. When this happens, the foot is turned inward and is occasionally followed by a “popping” sound. Walking is difficult and the foot shows immediate swelling. During this time, there is extreme pain.
Ankle Sprain
During his 35-year career in orthopedics, he has performed thousands of arthroscopic surgeries on severe shoulder, knee, and ankle injuries. His extensive knowledge of anterior cruciate ligament (ACL) injuries and foot and ankle conditions has enabled him to help patients return to their previous level of athletic activity both with and without surgery. He is among the few surgeons who is prepared to take on serious complications resulting from unsuccessful surgeries other physicians refuse to treat. He regularly treats patients with tears of other knee ligaments, tears of the meniscal cartilages, dislocation of the patella, and malalignment disorders.
Bunions Problems
While some are willing to make such a sacrifice for a designer name far more difficult to pronounce than the affliction they can cause, many simply do not realize that shoe choice can be such a significant contributor to bunion deformity and pain. But it is, in fact, all about shoe choice in both those genetically predisposed and those who are not – as rarely have bunions been identified in those who do not wear shoes.

Foot and ankle health is key to maintaining overall health and well being. Having a healthy foot and ankle helps to maintain the integrity of the musculoskeletal system – which is key to maintaining quality of life. Mobility is essential for longevity.
Flat Feet
Bunions are commonly hereditary, the result of several anatomic abnormalities such as instability of the first tarsometatarsal joint, an idiopathic gastrocnemius contracture (IGC) or Achilles tendon contracture, pes planus, also known as flat foot, and an excessive separation or angulation between the shafts of the first and second metatarsals, referred to as the intermetatarsal angle. The intermetatarsal angle can be thought of as a forefoot in which the metatarsal (foot bone) alignment resembles the metacarpal alignment of the hand. The presence of these abnormalities can be further agitated by the wrong footwear selection – and usually result in the most severe deformities.

Some of the common foot and ankle injuries and conditions we experience include ankle sprains, Achilles tendonitis and rupture, heel spurs and adult-acquired flat foot syndrome (AAFFS).

Metatarsalgia
Methods: This investigation was a prospective comparison of maximal ankle dorsiflexion, as a proxy for gastrocnemius tension, in response to a load applied to the undersurface of the foot in two healthy age, weight, and sex-matched groups. The patient group comprised thirty-four consecutive patients with a diagnosis of metatarsalgia or related midfoot and/or forefoot symptoms. The control group consisted of thirty-four individuals without foot or ankle symptoms. The participants were clinically examined for gastrocnemius and soleus contracture and were subsequently assessed for tightness with use of a specially designed electrogoniometer. Measurements were made both with the knee extended (the gastrocnemius under tension) and with the knee flexed (the gastrocnemius relaxed).
Plantar Fasciitis
Plantar fasciitis is one of the most common causes of heel pain. Approximately two million patients are treated for this condition every year.
Podiatric Surgery
Data Synthesis: Prevention of diabetic foot ulcers begins with screening for loss of protective sensation, which is best accomplished in the primary care setting with a brief history and the Semmes-Weinstein monofilament. Specialist clinics may quantify neuropathy with biothesiometry, measure plantar foot pressure, and assess lower extremity vascular status with Doppler ultrasound and ankle-brachial blood pressure indices. These measurements, in conjunction with other findings from the history and physical examination, enable clinicians to stratify patients based on risk and to determine the type of intervention. Educating patients about proper foot care and periodic foot examinations are effective interventions to prevent ulceration. Other possibly effective clinical interventions include optimizing glycemic control, smoking cessation, intensive podiatric care, debridement of calluses, and certain types of prophylactic foot surgery. The value of various types of prescription footwear for ulcer prevention is not clear.
