For myself, I developed plantar fasciitis after college while working in NYC in a sales job. I was still running 8-10 miles 5-6 times per week as I had done for years. You would think that alone would cause it. It wasn’t. It was the fact that I was now walking in shoes on concrete sidewalks all day long. Thankfully, a pair of custom orthotics was the answer for my problem. The pain calmed down almost immediately, I began to enjoy my running again, although I still hated the job at the time. To this day I still use orthotics in everything I put on my feet.

Recently, I attended a four day conference in NYC listening to lecturers from around the country on a variety of topics from Diabetes to surgery to nail fungus,, and everything in between. Two of the more interesting lectures to me, covered a topic that I have a great deal of interest in. The lectures were on Podo-Pediatrics. This is a branch of podiatry which focuses on any and all ailments of children’s feet.

FEET. It’s what we eat, drink and sleep each and every day. Okay, yuck! Not the most pleasant of thoughts, I agree. But wouldn’t you rather entrust yours or your own child’s feet to someone who has the training and specialization in this area? I know I would want my own daughter to see a specialist for whatever ails her. As always, if you have any questions regarding your overall health, please see your primary care physician. If you ever have any concerns of the health of your feet, please do not hesitate to contact your Podiatrist for a comprehensive foot, ankle and lower extremity evaluation.

Foreign Bodies: This includes glass, splinters, seashells, nails, needles etc. more common during this barefoot and sandal-wearing time of the year. If these items are not readily visible and do not come out easily, it’s best not to go digging. You may end up pushing it in deeper. It truly is best to be seen and have it removed by a Podiatrist. Unless, of course, you enjoy spending 4 hours in the emergency room.

X-rays, MRI’s and CT scans can be utilized to diagnose, treat and track the course of the condition. It can also, when performed early, curtail the inevitable as long as you heed the warning signs and advice of your trainer, coach or physician. What is the inevitable, you ask, if you don’t take care of Shin Splints early? Let’s see, a stress fracture of the lower leg, a serving of pain, quite a bit of time off from your favorite activity and a nice dose of frustration of your lack of progress to normalcy.

Bunions. The mere mention of the word for some remind them of their grandmothers wearing those big, black, boxy orthopedic shoes of yesteryear. I still, to this day, when I’m brave enough and dare to usher the word ‘bunion’ do I get “the look” and subsequent growl of, “I don’t have bunions”! I secretly love uttering those words. If not for anything else, just to see the look on some patients faces.

Okay, so what does skin cancer look like? Well, let me tell you a story. Last week I was at a conference in Atlantic City and attended more than a few lectures on skin conditions including benign and malignant lesions. All 3 lecturers had shown slides which they would flip back and forth between, challenging the audience to choose which freckle was benign and which was malignant. It would’ve been easier to pick a Triple Crown winner.
Dermatology
On one of the first dates that I was on with a girl a few years back, she noticed a big thick textbook in my car as I escorted her towards the passenger door. She asked me why I had a Dermatology textbook in my car. The response exited my mouth, I believe, quicker than my brain even processed the question. I said, “Because most of my patients have skin.” I feel that sometimes my smartass remarks are funny but in that instance I wasn’t so sure. Of course, you never know on those first few dates when trying to feel each other out. Well, she must have thought it somewhat comical because she eventually married me. Now when I say something to that affect I can see her head shake and her eyes roll.
Corns
Callouses and corns form when an accumulation of dead skin cells harden and thicken over an area of the foot, or anywhere else for that matter where there is increased pressure. This is one of the body's defense mechanisms against excessive pressure and/ or friction. Although they appear to be different in size, callouses and corns are made of the same material.
Calluses
Callouses have a tendency to cover a wider area although are not as deep as a corn. Corns, on the other hand, are smaller in diameter but are deeper and are usually more painful. The latter will develop over bony prominences (think outside of the pinky toe) while the former over an area that has more padding (think heels or ball of the foot). Some calluses have a deep seated core known as a nucleation, which is especially painful with pressure. This particular condition is referred to as an Intractable Plantar Keratosis or IPK for short. Some may refer these appearing as a corn within a callous since covers a larger area and has a deep core in the center.
Fungus
Nail fungus. Most of us have heard of, or about it. Some of us even have it. Yuk. Even the mere mention of it during an office visit with a patient or casual conversation with friends or even strangers for that matter, evokes a distorted facial look. I really do enjoy those looks when discussing this topic. Even now most of you may be wrinkling your nose a bit just thinking about it.
Lesions
Anyone who has raked leaves, worked in the garden or frequently uses hand tools will let you know how uncomfortable having callouses can be on their hands. Now let’s place those painful lesions on the weight-bearing surfaces of the feet and anyone with them will certainly let you know how they feel. Sometimes a callous will develop after a blister forms initially, then it opens or “dries’ out. What is left is an area of hardened, dry, dead skin-usually a callous.

Sports Injuries: Other than twists and sprains, sports injuries can include blisters, plantar fasciitis, neuromas and overuse injuries such as stress fractures and tendon and ligament injuries. Most of these can be alleviated by the use of properly fitting and sport specific shoewear. Proper training and conditioning is equally important in keeping injuries to a minimal.

A bunion can range from a small, painless bump at the big toe joint to an incredibly painful deformity with the big toe sitting either above or beneath the toe next to it. It can be uncomfortable even when barefoot and there is no pressure at that site. Some of the worst bunions I’ve seen, patients deny that they’re painful at all. What can cause the pain can be arthritic changes at the joint, shoe gear pressure, rubbing/ friction, diminished cartilage at either side of the joint, an infection or an inflammation of the bursal sac. This sac is similar to an air bag in a car that prevents further trauma at a particular site. This is different than a gout attack which also occurs at the big toe joint. Another issue altogether.

•3-4” Heels- Sure they look great and go well with that outfit but can exacerbate painful bunions or hammertoes and increase their deformity. Can cause Morton’s neuroma, an inflammation of a nerve usually due to ill-fitting shoes of this category.
Heel Spurs
The reason I bring this up is for a few reasons: One, if you’ve over-trained for a marathon (twice) I’m right behind you. If you’ve walked onto a playing field with a broken bone or two, I am right beside you. If you’ve had to stop certain activities because of a pain in the foot so severe it had brought you to tears. Oh man, don’t I know it. Not so much from the pain. It’s just the utter frustration of NOT being able to run, cycle, sit, sleep or even walk for that matter, like a normal human being. Even if it is only a short trip from the car to the front door or from the bedroom to the bathroom at 3am. If it’s a broken toe, tendonitis, heel spur, ingrown nail, neuroma, a muscle tear or a myriad of other annoying, frustrating improbable setbacks, I truly have been in your shoes. Sorry, no pun intended, but it did seem to fit.

If you’ve read any of my previous articles you’ll notice one thing that may not be evident in articles written by other healthcare authors. Most of what I’ve treated patients for I have, at one time or another suffered with that problem myself...broken toes, foot and ankle, yep. Ingrown nails, neuroma, tendonitis, plantar fasciitis and heels spurs, oh yeah I’ll never forget those debilitating problems as long as I live.
