Superficial thrombophlebitis or superficial vein thrombosis with phlebitis is a condition where superficial veins thrombose or clot can cause inflammation and induration (hardening, thickening) of the overlying skin. There is also associated erythema (redness) and palpable warmth over the course of the vein. Patients complain of pain upon touching the thrombosed cords of veins and may feel warmth upon touching the area that is involved with phlebitis. They often go to emergency rooms for evaluation, believing that the redness represents infection in the lower extremity. In turn, emergency room physicians may incorrectly treat the problem with antibiotics, prolonging the duration of symptoms and skin complications that result from this inflammatory process.

This has been a highly successful program. As well, he has donated surgical instruments for use in a vascular and ophthalmology center of excellence in the Republic of Armenia and the province of Nagorno Karabakh.

Researchers at Harvard published a study in May 2008 in the Archives of Internal Medicine that investigated the association of particulate air pollution to deep vein thrombosis, i.e. deep vein clots). It is well accepted that air pollution is linked to heart disease and stroke but the association with deep vein clots was unknown until this study was published. Dr. Baccarelli and colleagues studied this phenomenon in subjects living in the Lombardy region in Italy. They examined the association of exposure to particulate matter of less than 10 microns. Using 870 patients and 1210 controls, they concluded that long-term exposure to particulate air pollution is associated with altered coagulation function and deep vein clot formation risk. Of note (and perhaps too detailed for this discussion), each increase of 10 microg/m3 in particular matter in air was associated with a 70% increase in deep vein clot (DVT) risk. So, avoid living in cities with a significant burden of air pollution ro reduce this risk. By the way, living in low air pollution areas will also reduce your chances of developing asthma, heart disease and stroke.

A very detailed and thorough history and examination is performed to rule out comorbid conditions that contribute to the poorly healing or non healing wound. These include conditions such as thyroid disorders, diabetes, kidney insufficiency and failure, among others. As well, coagulation factor disorders are sought out and investigated by special blood tests. The spectrum of blood tests and coagulation factor studies is determined and individualized by Dr. Karamanoukian at the time of consultation. All of these studies are covered by medical insurance.

What are the symptoms of Chronic Venous Hypertension? Symptoms of chronic venous hypertension may include swelling, pain, tenderness, venous stasis skin changes, or venous leg ulcers. Diagnostic evaluation includes lower extremity venous color duplex ultrasound and sometimes other imaging such as CT or MRI scans specially performed to evaluate the veins of the legs, pelvis, or abdomen.

Symptoms of May-Thurner Syndrome All people with May-Thurner syndrome will not have symptoms secondary to the compression of the left common iliac vein. Sometimes, it is discovered by accident when imaging (particularly CT scan or MRI) is performed for other reasons. Most of the time it is discovered during the work-up of a DVT of the left leg. Symptoms can include pain and/or swelling. May-Thurner syndrome occurs more commonly in women aged 20 to 50 years.
Chemotherapy
Moderate risk factors have an odds ratio between 2 and 9, indicating an increased risk by 2 to 9 fold. These include arthroscopic knee surgery, the presence of central venous lines, chemotherapy, congestive heart failure, respiratory failure, hormone replacement therapy, cancers, the use of oral contraceptive agents, paralytic stroke, being pregnant, previous episodes of venous thromboembolism and thrombophilia.

Dr. Hratch Karamanoukian was administrative Chief Resident in Cardiac Surgery from 1998 - 1999 and received the George Schimert award for outstanding graduating resident.
Wound Care
Very rarely are patients able to work as these open wounds require constant care and cause significant disability. Over 80 % of patients with venous stasis ulcers have limited mobility. According to studies reported in the medical literature, in terms of quality of life, the open wounds cause more concern for patients than symptoms of pain, itching, anxiety, social isolation and reduced ability to perform their daily routine. At least, these wounds require daily or twice daily wound dressing changes. In advanced healthcare environments and systems, patients with venous stasis ulcers are seen by home health care nurses at least once weekly. Physicians who are specialized in wound care also supervise the evolution of these healing wounds once monthly and more frequently when they are infected or have significant necrotic tissue. Dr. Karamanoukian has the experience to recognize and treat infections that are contributing to poorly healing or non-healing venous stasis ulcers.
