Per CDC recommendations and the Minnesota Department of Health we are now employing telemedicine services for all office visits until further notice. Injections, medication pickup, paperwork pickup are still offered on site. MASKS ARE REQUIRED for on site visits, as well as, proof of vaccination or negative covid test taken within 48 hours of your visit. Please use our sanitation station when entering the building. Your temperature will be taken and COVID-19 screening questions will be administered. Appointment times are staggered so that we can follow social distancing requirements. We are administering these practices for the safety of our patients and staff. Thank you for your patience and flexibility.

Attention Deficit Hyperactivity Disorder (ADHD)
Jacob Moore received his BA from St. Johns University and his MPAS from Augsburg College. His clinical approach involves medication management in an outpatient setting. His clinical interests include depression, anxiety disorders, bipolar, and ADHD. When Jacob is not working, he is an avid sports enthusiast, enjoys boating, yard games, long walks with his dogs, and traveling to new places.
