The building features a secured, covered patio and cozy lounge areas that provide the perfect setting for either quiet time or social activities. At The Laurels of DeKalb we provide quality services to both short-term and long-term guests. Our motto, Sooner, Safer, Stronger, signifies our goal for the way each of our short-term rehab guests transitions from hospital to home. We also offer home-based therapy after discharge.

General Practitioners

Primary Care
Yes! We believe it is every Guest’s right to choose their Attending Physician. The physicians that see our Guests must complete the Facility’s credentialing process which includes having a current license to practice medicine in the State where the Facility is located.
Immunizations
Per CMS, indoor visitation is currently allowed for all residents, and vaccination status does not affect the ability to visit. However, visitors are still required to follow facility policies, wear appropriate PPE, and observe physical distancing during their visit.

Internal Medicine
Dr. Ryan C Rolf is an Internal Medicine Specialist in Fort Wayne, Indiana. He graduated with honors from Indiana University School of Medicine in 2000. Having more than 18 years of diverse experiences, especially in INTERNAL MEDICINE, Dr. Ryan C Rolf affiliates with many hospitals including Parkview Regional Medical Center, Dekalb Health, Lutheran Hospital of Indiana, and cooperates with other doctors and specialists without joining any medical groups.

Multiple Sclerosis

Radiology
Medicare Part B is the "supplemental medical insurance" portion of Medicare. Part B covers physician services, outpatient radiology and laboratory services, therapy, medically necessary ambulance transport, certain supplies and equipment, and a few other services not covered by Part A. Part B does not cover most outpatient medications. Part B is a voluntary program that requires eligible individuals to enroll and pay a monthly premium. Most people with Part A choose to enroll in Part B as Part A and Part B are designed to provide a comprehensive health plan when used together.

The Laurels of DeKalb employs a team of highly qualified medical professionals, including a Medical Director, Registered Nurses, Restorative Care Nurses, a Wound Care Nurse, state tested Certified Nursing Assistants, Physical Therapists, Occupational Therapists, and a Speech Therapist. Our on-site Rehab, Dietary, Social Work, and Activities departments provide personalized and compassionate care to each guest. From the moment you walk through the front doors, the warmth and caring of the staff will begin to gain your trust and confidence, as they strive to exceed your expectations.

Physical Therapists

Physical Therapy
Duane graduated from Tri State University with a degree in Criminal Justice in 1997 but quickly realized physical therapy and rehabilitation was his passion. As a result, he graduated from Ivy Tech in Muncie with an associate's degree in physical therapy in 2001. He has over 20 years of therapy experience with the long-term care and skilled nursing populations.
