Fall General Surgery and Ashland Memorial Medical Center is pleased to announce the recent acquisition of the da Vinci Xi Surgical System. The da Vinci Xi System was designed with the goal of further advancing the technology used in minimally invasive surgery. The System can be used across a spectrum of minimally invasive surgical procedures and has been optimized for multi-quadrant surgeries in the areas of gynecology, urology, thoracic, cardiac and general surgery.

Endoscopy
Because symptoms can be disabling, many patients are referred for surgical evaluation. Prior to recommending laparoscopy, multiple diagnostic studies are usually performed; i.e., CT scans, MRI, upper/lower endoscopy, etc. Commonly, despite negative studies, many patients will continue to be symptomatic.

Ultrasound
How does it work? During the procedure, a trained clinician fills a syringe with the medical adhesive, which is inserted into the VenaSeal™ closure system’s dispensing gun that is attached to a catheter. The catheter is advanced into the diseased vein under ultrasound guidance. The catheter is placed in specific areas along the diseased vein and the clinician conducts a series of trigger pulls to deliver the medical adhesive. Compression is applied to the leg during the procedure.
MRI
MRI and EUS are both quite accurate in finding stones in the bile duct. ERCP can be used to locate and sometimes remove stones in the bile ducts. The doctor inserts an endoscope (a long, flexible, lighted tube with a camera) down your throat, through the stomach and into the small intestine. The doctor injects an x-ray dye to help better see the bile ducts and find gallstones. If the stones can be removed safely, they are captured in a tiny basket and removed with the endoscope.

General Surgery
Fall General Surgery has the privilege of treating, among many things, chronic abdominal pain. By “chronic” we refer to the existence of symptoms over several weeks, months, or even years.
Minimally Invasive Surgery
In the last decade, we have made great advancements in surgery. One of the most noticeable advancements is robotic-assisted minimally invasive surgery using the da Vinci Surgical System. For gallbladder removal, it can be performed with one small incision in the bellybutton.
Hand Surgery
Gangrene
Stroke, transient ischemic attack, myocardial infarction, and impaired cardiac function have been reported in close temporal relationship with polidocanol administration. These events may be caused by air embolism when using the product foamed with room air (high nitrogen concentration) or thromboembolism. The safety and efficacy of polidocanol foamed with room air has not been established and its use should be avoided. Accidental injection into an artery can cause severe necrosis, ischemia or gangrene. Care should be taken in intravenous needle placement and the smallest effective volume at each injection site should be used. If intra-arterial injection of polidocanol occurs, consult a vascular surgeon immediately. After the injection session is completed, apply compression with a stocking or bandage, and walk for 15-20 minutes. Your healthcare provider will provide monitoring during this period to treat any possible anaphylactic or allergic reactions. Maintain compression for 2 to 3 days after treatment of spider veins and for 5 to 7 days for reticular veins, or as directed by your Healthcare Provider. For extensive varicosities, longer compression treatment with compression bandages or a gradient compression stocking of a higher compression class is recommended. Post-treatment compression is necessary to reduce the risk of deep vein thrombosis.
Vascular Surgery
Results from the European Sapheon Closure System Observational ProspectivE (eSCOPE) study as presented in the Journal of Vascular Surgery demonstrate a cumulative closure rate of 88.5 percent and improvement in quality of life scores at 36 months.
Robotic Surgery

Da Vinci ® Surgery with Single-Site ® Instruments and accessories is cleared for use in gallbladder removal, and for hysterectomy and ovary removal for benign conditions. Patients who bleed easily, have abnormal blood clotting, are pregnant or morbidly obese are not candidates for minimally invasive surgery, including da Vinci Surgery with Single-Site ® Instruments. The safety and effectiveness of this device for use in the performance of general laparoscopic abdominal surgery procedures have not been established.
Laparoscopy
For some of these patients, diagnostic laparoscopy may be recommended and, in many of these patients, pathology can be identified and corrected. Since laparoscopy utilizes only three to four tiny incisions, post-op pain is minimal.

Laparoscopic Surgery
Because the abdominal muscles are not cut, the patient may feel less pain after surgery, have a faster recovery and smaller scars with laparoscopic chole compared to open chole. The patient can usually leave the hospital the same or next day and return to normal activities within a few days after laparoscopic surgery.

Indoor tanning first became popular in the 1970’s. Today 20,000-25,000 salons are listed in the yellow pages marketing to 22 million clients per year. The use of tanning devices has now been clearly linked to the development of the two most common forms of skin cancer: basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). Such devices increase the risk of developing these cancers by 1 1/2 to 2 1/2 times. The incidence of developing malignant melanoma, the most dangerous skin cancer, is also increased.
