Arthritis
As an introduction, for decades steroids (cortisone) has been used to treat the swelling and pain of osteoarthritis. Arthritis can be mild (Grade 1-2), with softening of the cartilage that covers the end of the bone, to moderate (Grade 2-3), to severe, when the cartilage is worn down to bone (Grade 4). It has been very successful in relieving swelling and offers short term improvement in pain, but long-term relief is not as predictable. It is low cost, easy to administer during an office visit, and does not require lengthy authorization and paperwork from the insurance companies. However, repetitive use of steroid injections is not healthy for the articular cartilage in any joint or tendon and can increase the risk of infection should surgery become necessary. It is usually restricted to at most 2-3 a year in most cases. (This is different if the patient has rheumatoid arthritis or inflammatory arthritis, which is a different problem); Many insurance companies now require a trial of a cortisone injection before use of HA. HA has also been increasingly NOT covered by insurance companies as well and does not last as long as PRP. PRP is not covered by insurance companies, except some Workers Compensation companies..

Most diseases and injuries can be treated by a variety of methods depending on the individual circumstances of the patient. For example, arthritis of the knee might be treated first with modification of activities, weight loss, over the counter anti-inflammatory medications, physical therapy, bracing, corticosteroid injections, and hyalouronic (lubricant) injections. In some cases, knee arthroscopy may be helpful and in the most advanced stage of knee arthritis, total knee replacement is the most reliable method for decreasing pain and improving function. Itâ€™s important to weigh the pros and cons of your various treatment options before deciding which approach is best for you.
Primary Care
Although the operation involves your knee or shoulder, your entire body will react to the anesthesia and surgery. Your general medical condition is obviously important. If you are younger than 50 and in good health, routine laboratory tests are not necessary. You will need to see an anesthesiologist before going into surgery. If you have any medical problems, you will need to see a primary care doctor with whom we work with will or your regular doctor prior to surgery. Dr. Anderson feels this will provide you with the best medical care.
Emergency Care
If there is an urgent need or a postoperative question after office hours call 415-923-3029 to have Dr Anderson paged. If you feel your problem is an emergency either call 911 or proceed to the nearest emergency room. Medications are not refilled after hours or on weekends. If you are canceling or changing an appointment or have other non-urgent questions please leave a message on our voice mail.

Anxiety

Ultrasound
The final syringe of platelet-rich plasma will contain approximately 1-2 teaspoons of fluid. With the guidance of an ultrasound probe, the PRP will be guided into the proper location, based on the nature of the injury being treated.
MRI
We do not store outside MRI or X-ray films in the office. Please contact the facility where these were performed directly.
X-Rays

General Surgery
Many patients have found the benefit of guided imagery to help reduce anxiety, decrease pain and shorten their recovery. Studies show that patient who are calm and relaxed recovery more quickly from surgery. Orthopedics
I just attended 2 meetings on the use of Biologics in Orthopedics. Many of the world's best thought leaders and clinicians shared their experiences and opinions on the current indications and use of these biologics.
Arthroscopy
The use of PRP (platelet rich plasma) is a very straightforward procedure that involves taking blood in the office, spinning it down, concentrating the platelets and injecting it into arthritic joints. It’s best to use PRP that is low in white cells so there is less inflammation for joints. (yellow PRP, not red). The "red" PRP may be better for injecting tendons as having inflammation in a tendon will signal healing. There have been several excellent studies that have shown that PRP is superior and lasts longer than cortisone or hyaluronic acid or lubricant injections. (Arthroscopy, Dai et al., Vol 33. No 3 March 2017.
Joint Replacement
We recommend the Lipogems for patients that have had part of their meniscus removed as part of our approach to avoiding progression of their arthritis, mild-moderate arthritis, or in cases of patients wanting to try alternatives to total joint replacement.
Orthopedic Surgery
Dr. Lesley Anderson has been a specialist in complex knee and shoulder problems since 1984. She was born and raised in Pennsylvania where she actively participated in sports throughout college, specifically lacrosse and field hockey. She developed her interest in treating injuries of the knee and shoulder through her own personal surgeries as a teenager, when orthopedic surgery was primitive compared with current techniques.
Arthroscopy
This surgery is done using arthroscopic technique. Instead of a large incision Dr. Anderson use 3 small (about 1/2”) incisions. During an arthroscopy, a small camera, called an arthroscope, is inserted near the joint through a small (usually one-quarter inch) incision. The arthroscope is attached to a video monitor to allow Dr. Anderson to look inside your joint. She inserts special instruments that allow the removal of scar tissue, bone, and to perform the repair through the other two incisions.

Knee Replacement
She has been at the forefront in developing biologic resurfacing techniques of the knee for the past 20 years as an alternative to knee replacement. This has included meniscus transplantation, fresh allografts for treating large cartilage defects of the knee, osteochondral autografts for smaller cartilage defects to autologous cartilage implantation (MACI) for cloning the patients' own cartilage cells for treating cartilage defects. She volunteered for the Olympic Games in LA in 1984.

Physical Therapy
We have been using PRP for over 10 yrs. and MFAT for over 6 years. Our experience has been very positive, and I continue to be an advocate of the use of these biologics in well informed patients who have not responded to traditional methods, such as anti-inflammatories, physical therapy or cortisone injection and as an alternative to surgery. It is important to note that PRP and MFAT will not heal a joint that is having mechanical symptoms such as giving way, locking or instability.
Tennis Elbow
PRP was initially used for tennis elbow, and continues to show excellent results in chronic tennis elbow, Achilles tendinosis, hip "bursitis" and a bit less predictable results in patellar tendinosis. This is an option when all non-interventive treatment fails. It is important to remember that PRP is NOT the first treatment that should be used. It has also shown benefit in acute muscle tears or injuries.
