Weight ManagementBreast augmentation, technically known as augmentation mammoplasty, is a surgical procedure to enhance the size and shape of a woman’s breast, and is performed for a number of reasons. Some women feel that their breast size is too small for their body contours, while others may be seeking to turn the clock back on the effects of age and gravity. A lack of symmetry and living with the discomfort of ill-fitting clothing that comes as a result; lost volume due to weight loss or pregnancy; coping with missing or misshapen breasts as a result of trauma, disease (such as cancer), or congenital abnormalities.
MammographyAll surgical procedures come with some measure of risk, and the most common risk in breast augmentation is that of capsular contracture, which occurs when the capsule or scar around the implant begins to tighten. This can cause the breast to feel hard, and can exacerbate difficulties with mammography. Capsular contracture can be treated in several ways, and sometimes requires either removal or “scoring” of the scar tissue, or perhaps removal or replacement of the implant.
Radiation TherapyNo matter what approach is taken, keloids have a stubborn tendency to recur, sometimes even larger than before. To discourage this, the surgeon may combine the scar removal with steroid injections, direct application of steroids during surgery, or radiation therapy. Or you may be asked to wear a pressure garment over the area for as long as a year. Even so, the keloid may return, requiring repeated procedures every few years.
UltrasoundThe ultrasound energy used to liquefy fat cells and ease the process of removal in the UAL technique can also cause burns to the skin and even though UAL has been performed successfully on several thousand people worldwide, the long-term effects of ultrasound energy on the body are not yet known. The high doses of anesthetic injected into the area being treated in the tumescent and super-wet techniques can cause toxicity if levels and absorption are not managed properly. And even though a well-trained surgeon and a state-of-the art facility can improve your chance of having a good result, there are no guarantees. Risks increase if a greater number of areas are treated at the same time, or if the operative sites are larger in size, since removing more tissue means a longer procedure with more opportunity for complications to arise.
MRIInjury and even normal compression and movement of the breast can cause rupture and leakage. Saline-filled implants simply drain into the body, where the salt-water is absorbed and expelled naturally. Silicone-filled implants may not deflate, and a rupture may not even be noticeable without ultrasound or MRI. For this reason, a woman with silicone breast implants is advised to visit her plastic surgeon annually to assess that her implants are functioning well. An ultrasound exam or MRI screening can assess the condition of breast implants. After 3 years it is recommended that all silicone implants be properly screened.
RadiologyReconstruction has no known effect on the recurrence of disease in the breast, nor does it generally interfere with chemotherapy or radiation treatment, should cancer recur. Your surgeon may recommend continuation of periodic mammograms on both the reconstructed and the remaining normal breast. If your reconstruction involves an implant, be sure to go to a radiology center where technicians are experienced in the special techniques required to get a reliable x-ray of a breast reconstructed with an implant.
GlaucomaA few medical conditions make blepharoplasty more risky. They include thyroid problems such as hypothyroidism and Graves’ disease, dry eye or lack of sufficient tears, high blood pressure or other circulatory disorders, cardiovascular disease, and diabetes. A detached retina or glaucoma is also reason for caution; check with your ophthalmologist before you have surgery.
General SurgerySince your surgeon’s education and training have helped to form his or her surgical judgment, take the time to do some background checking. An important consideration is choosing a doctor certified by the American Board of Plastic Surgery (ABPS). To be certified by the ABPS, a doctor must practice surgery for two years and pass comprehensive written and oral exams, graduate from an accredited medical school and complete at least five years of additional residency – usually three years of general surgery (or its equivalent) and two years of plastic surgery. By choosing an ABPS certified plastic surgeon, a patient can be assured that the doctor has ample preparation.
Ear SurgeryEar surgery is usually performed as an outpatient procedure in a hospital, a doctor’s office-based surgical facility, or a freestanding surgery center. Occasionally, your doctor may recommend that the procedure be done as an inpatient procedure, in which case you can plan on staying overnight in the hospital.
Orthopedic SurgeryMy first encounter with Dr. Hallak was through a clinic where I had gone for help with a wound on my hip that had not healed following orthopedic surgery many months before. Dr. Hallak sometimes assisted the clinic by performing plastic surgery as the final step in the healing process of such wounds, and I was very fortunate to receive his services. I was greatly impressed with his caring attitude toward me, and with his skill in healing my wound. Indeed, today my hip is left with only a minor scar.
Reconstructive SurgeryReconstructive surgery is performed on abnormal structures of the body, caused by congenital defects, developmental abnormalities, trauma, infection, tumors or disease. It is generally performed to improve functions, but may also be done to approximate a normal appearance. Reconstructive surgery is generally covered by most health insurance policies, although coverage for specific procedures and levels of coverage may vary greatly.
Labor and DeliveryIn cases where only a small amount of fat involved and the number of body sites are limited, local anesthesia, which numbs only the affected areas, can be used. The local anesthetic can be used along with intravenous sedation to keep you more relaxed during the procedure, if you prefer. Regional anesthesia, such as an epidural block like those commonly used in childbirth, can be a good choice for more extensive procedures. Some patients prefer general anesthesia, particularly if a large volume of fat is being removed. In such cases, a nurse anesthetist or anesthesiologist will be called in to make sure you are completely asleep during the procedure.
Chemical PeelsSun exposure, acne or just getting older can leave your skin tone uneven, wrinkled, spotted or scarred. If you want your skin to look smoother and younger, consider a chemical peel, which uses a chemical solution to smooth the texture of your skin by removing the damaged outer layers.
EndoscopyIf a complication should occur during an endoscopic forehead lift, your surgeon may have to abandon the endoscopic approach and switch to the conventional, open procedure, which will result in a more extensive scar and a longer recovery period. To date, such complications are rare — estimated at less than 1 percent of all endoscopy procedures.
BotoxThe cosmetic form of botulinum toxin, often referred to by its product name Botox®, is a popular non-surgical injection that temporarily reduces or eliminates frown lines, forehead creases, crows feet near the eyes and thick bands in the neck. The toxin blocks the nerve impulses, temporarily paralyzing the muscles that cause wrinkles while giving the skin a smoother, more refreshed appearance. Studies have also suggested that Botox is effective in relieving migraine headaches, excessive sweating and muscle spasms in the neck and eyes.
Eyelid SurgeryThe best candidates for eyelid surgery are men and women who are physically healthy, psychologically stable, and realistic in their expectations. Most are 35 or older, but if droopy, baggy eyelids run in your family, you may decide to have eyelid surgery at a younger age.
LiposuctionAn arm lift, also known as brachioplasty, is a surgical procedure to remove loose skin and excess fat deposits in the upper arm. With age, upper arm skin can become loose and flabby. Plastic surgeons will discuss brachioplasty with patients who want to tighten this skin and look as good as they feel. In some cases your plastic surgeon may suggest that liposuction be used alone or in conjunction with an arm lift to remove excess fat in the upper arms.

Breast ReductionBreast reduction, technically known as reduction mammaplasty, is designed for such women. The procedure removes fat, glandular tissue, and skin from the breasts, making them smaller, lighter, and firmer. It can also reduce the size of the areola, the darker skin surrounding the nipple. Face LiftWhen a facelift is performed by a qualified plastic surgeon, complications are infrequent and usually minor. Still, individuals vary greatly in their anatomy, their physical reactions, and their healing abilities, and the outcome is never completely predictable.
BlepharoplastyThe minor complications that occasionally follow blepharoplasty include double or blurred vision for a few days; temporary swelling at the corner of the eyelids; and a slight asymmetry in healing or scarring. Tiny whiteheads may appear after your stitches are taken out; your surgeon can remove them easily with a very fine needle.
AbdominoplastyPatients whose intent is to lose significant weight should postpone the surgery, as should women who plan future pregnancies. Vertical abdominal muscles that are tightened during surgery can separate again during pregnancy. Your doctor may recommend against abdominoplasty if you have scarring from previous abdominal surgery, or caution you that scars could be unusually prominent.
Nose SurgeryBe sure to tell your surgeon if you’ve had any previous nose surgery or an injury to your nose, even if it was many years ago. You should also inform your surgeon if you have any allergies or breathing difficulties; if you’re taking any medications, vitamins, or recreational drugs; and if you smoke.

GynecomastiaThe initial consultation with your surgeon is very important. Your surgeon will need a complete medical history, so check your own records ahead of time and be ready to provide this information. If a medical problem is the suspected cause, you’ll be referred to an appropriate specialist.
DermabrasionSome facial scars can be softened using a technique called dermabrasion, a controlled scraping of the top layers of the skin using a hand-held, high-speed rotary wheel. Dermabrasion leaves a smoother surface to the skin, but it won’t completely erase the scar.
Facial ImplantsPlastic surgeons use facial implants to improve and enhance facial contours. Frequently, these implants will help provide a more harmonious balance to your face and features so that you feel better about the way you look.
Forehead LiftRemember, a forehead lift can enhance your appearance and your self-confidence, but it won’t necessarily change your looks to match your ideal or cause other people to treat you differently. Before you decide to have surgery, think carefully about your expectations and discuss them in detail with your doctor.
Physical TherapyCorrecting a contracture usually involves cutting out the scar and replacing it with a skin graft or a flap. In some cases a procedure known as Z-plasty may be used. And new techniques, such as tissue expansion, are playing an increasingly important role. If the contracture has existed for some time, you may need physical therapy after surgery to restore full function.
