        STAY AWAY from this facility. It is a dungeon with very few staff that truly care about the patients. The rooms are messy, furniture is falling apart. The food is atrocious, not appropriate for patients in need of solid nutrition for healing. 48 hours after her admission to Heartland-Plymouth, my Mom was still wearing the same hospital gown she had on when they transferred her from the hospital. I walked in to find her in the hallway, hair uncombed and sitting in a dirty gown looking like a home-less person. She was in I cleaned her up, I changed her clothes and put her back in her wheel chair. Promptly arranged a transfer. No help with that either-the SW did only what she had to do. I asked for a wheel-chair van to transfer her and I was told that they didn't know how long it would take and if she could go that day. I asked for help to get her in my car so I could take her-again no assistance. I called my husband and he drove 30 minutes to come and assist me with transferring my mom to my car. We drove away quickly and did not look back. Horrible experience. I have heard these stories but never thought I would live it. Stay away!! Heartland, you should be ashamed of yourselves.
