All Pro Medical Supplies Inc

I visited in order to purchase a medical device and was asked to complete forms including social security number. After completing the form and submitting insurance card I was informed there was no coverage. Ok. I was then told I could not leave the store with my own personal information which included the social security number. A man working there actually grabbed the form from my hands (5’6” 180 lbs. and obvious hair plugs) apparently because the form had a different business name at the top (not “all pro” but “Integra?”). He risked a physical confrontation to hide something (or keep the social security number). You have to decide for yourself but I’m not going back.
