        Gave you my information at very first appointment, along with repeating giving the information of medication list at the first consultation with Psych Doctor. The paperwork I gave you included all medications/dosage/ diagnoses etc. Then You give me papers for a bunch of tests I have to go complete before you will prescribe my medication. So I jump through the hoops to do that, THEN, I FINALLY get an appointment with the doctor, and I'm told you DO NOT prescribe 2 out of 3 of my medications!! Your practice has now put me and my health in jeopardy. Nobody had any interest in telling me that they cannot or will not prescribe the medication I've been taking for the last 5 years. This is some serious negligence.
